Yin Yang: the concept of duality forming a whole can be extended to CPB/ECMO/ECLS and inflammation. Advances in perfusion have led to wider acceptance of perfusion technology beyond extracorporeal membrane oxygenation and extracorporeal life support to percutaneous minimised perfusion systems to transport of critically ill patients. A better understanding of the technology and the haematological effects on the blood have led to the development of newer indications. Together with the advantages of minimally invasive cardiac surgery to standard extracorporeal life support usage in epidemiological support for bird flu epidemics, there has also been much more interest in the effect of haemoglobin, anaemia and haemodilution and the impact of perfusion and postoperative success and outcome. Perfusion technology continues to advance and now is amongst one of the most technologically advanced sub-specialities in medicine. Understanding the haematological effects and the impact on various body organs is paramount to extending indications and usage. De Jong reviews the different perfusion techniques for minimally invasive valve procedures and Papadopoulos analyses risk factors associated with adverse outcomes following extracorporeal life support. As with advances and wider applicability, understanding the risk factors and evaluating adverse postoperative outcomes will inevitably lead to better success. outcome and prevalence of preoperative anaemia in patients having first time cardiac surgery. Hilberath measures blood volume by haemodilution and association with valve disease. Pekel looks at the effects of CABG on retinal vascular calibre, ocular pulse amplitude and retinal thickness in an emerging concept of a window visualising the effects of CABG on microvascular structures. Clinical outcome and blood transfusion after infant cardiac surgery with routine use of conventional ultrafiltration is evaluated by Golab. Tagaya proposes an alternative haemodialysis method using a haemoconcentrator during cardiopulmonary bypass. As time passes and science progresses, we learn and increase our knowledge, with better outcomes for our patients and find ingenious methods of treatment by extending the boundaries within our realm.
