Schenectady, N.Y. Occasionallyit is necessaryto removea double abutment fixed partial denture where one abutment is looseand the other is firmly fixed. Judicious removal of the restoration from the fixed abutment sometimesallowsthe restoration to be preserved, which savesthe expenseof a new prosthesis. Various methods such as the reverse mallet, Atwood crown and bridge remover (Atwood Industries, Tarzana, Calif.), and the Parke11crown remover (Parke11Products, Inc., Farmingdale, N.Y.) have beenusedwith somesuccess. The following technique is often successful.

3. Cut the Para-Post sothat 2 mm protrudes above the occlusal surface (Fig. 1). 4. Position a Kelly clamp under the restoration to provide the occlusalforce, and place your thumb over a heavy mixing spatula, which should cover the post and act as the fulcrum over which pressureis applied with the thumb (Fig. 2). Pressureon the thumb and simultaneouslifting pressure on the Kelly clamp create a considerableamount of force. The 2-mm length of the post prevents the operator from exerting a sudden, excessiveforce when the crown breaks loosefrom the abutment.

1. Prepare accesswith an appropriate bur at the center of the occlusal surface. The accessshould just break through metal and shouldstop assoonastooth structure or cement is reached. 2. Widen the accessopening to accommodate a loosefitting 44-5 post (Para-Post System, Whaledent, New York, N.Y.).

ADVANTAGES

1. A controlled force is applied. 2. The force doesnot torque the tooth. 3. Marginal integrity is preserved. 4. No specialequipment or kit is required. DISADVANTAGE

1. Accessis sometimesdifficult on posterior teeth. aPrivate practice. 10/4/38203
