The Year in Review: Perioperative Medicine 2013

Nathaen S. Weitzel, MD1 The year 2013 was busy and productive for both Seminars in Cardiothoracic and Vascular Anesthesia (SCVA) and the scientific community of perioperative medicine. With that in mind, I have recruited a series of articles aimed at selecting some of the highlights from the year within multiple disciplines, serving to provide readers with an overview of key publications to refer back on. Obviously, with the large number of publications in the scientific community annually, it is impossible to present a comprehensive review—thus there is inherent bias in what was selected. I recruited specific expert authors from a variety of disciplines to present these works, and am pleased with the outcome. The Year in Review section of the March SCVA is a compilation of articles from Cardiothoracic Anesthesiology, Cardiac Surgery, Thoracic Surgery, Congenital Heart Disease, and Regional Anesthesiology. Each of these articles is an excellent read, reviewing selected topics and providing a nice group of key articles to read about further within each area. I want to thank Drs Hilberath, Muehlschlegel, Reece, Cleveland, Twite, Ing, Bennett, Weyant, and Fiegel for their hard work on these articles. The March issue of SCVA also contains a full complement of review and clinical challenges articles to start off 2014. Drs Chau and Slinger have authored an excellent review titled “Perioperative Fluid Management for Pulmonary Resection Surgery and Esophagectomy.” This article serves as an update on fluid management for both esophageal operations and lung resections, a key and highly debated topic. Drs Bottiger, Esper, and StaffordSmith have authored the second excellent article titled “Pain Management Strategies for Thoracotomy and Thoracic Pain Syndromes,” which dovetails nicely with the previous article. This review covers recent advances in pain management for thoracic operations along with a clinical management strategy based on current evidence.

In the final review article, Dr Eugene Hessel has authored a comprehensive review titled “Management of Patients With Implanted Ventricular Assist Devices for Noncardiac Surgery: A Clinical Review.” The number of ventricular assist devices implanted annually in the United States is increasing yearly, with more and more patients obtaining these devices for destination therapy. This group of patients will therefore begin to present for noncardiac surgery with increasing frequency. There is no consensus at this time as to whether patients with ventricular assist devices should be cared for by cardiac anesthesiologists, or generalists, thus it is imperative that we all begin to gain a comfort level with these devices. Second to this is the perioperative care of these patients, as they will need appropriate pre- and postoperative care based on the presence of the device. Dr Hessel has reviewed in great detail the current evidence for this group of patients and laid out clinical recommendations that can be followed by any practitioner. Finally, the March issue features 2 intriguing clinical challenges articles. The first article, authored by Drs Setty, Bello, and Leff, describes a novel approach for using epicardial ultrasound to identify the coronary arteries on the surgical field. The second article covers anesthetic and surgical consideration for cardiac surgery in the setting of Pemphigus vulgaris, authored by Drs Takahashi, Rhee, Rakasi, Yildirim, Silvay, and Reddy. Both articles are well written and interesting clinical challenges. 1
