pH stick (enclosed with the kit) is stroked over it and the pH checked on a colour chart. The length measured from the incisors and the pH can then be closely correlated with anatomical localisation (i.e., oesophagus, distal or more proximal). All three patients with positive pH probe tests after acid instillation into the stomach had pH 3 or less at a string length highly suggestive of more proximal intracesophageal localisation. This test may provide an accurate, simple alternative to other methods for detection of acid reflux. a

SIR,-Obese patients have higher serum-F.F.A. concentrations than do normal-weight controls, and this is believed to be an effect of lipolysis in the enlarged pool of body fat. We have previously shown significantly increased hepatic synthesis-rate of triglyceride (T.G.) concomitant with increased T.G. content in liver in severely obese patients (among them the ones in the study Dr Jenkins comments on) at the time of jejunoileostomy.2 When the patients achieved a stable level of body-weight, on average 19 months after bypass, there was a statistically significant decrease in serum-F.F.A.
