220 Total individual cases held: 1,000 % cases associated with fresh-frozen tumor biospecimens: 65% % cases with paraffin embedded biospecimens: 80% % cases with blood and blood derivatives: 93% In your view, what are the most valuable/highest priority cases in your biobanks? The most valuable cases are those that have more than 5 years follow-up data associated with them. This should include what treatment these patients have had so that appropriate outcome data can be incorporated in to any molecular analysis. What are your major challenges? The major challenges we face are accurate patient information that includes a range of indicators that are followedup on a regular basis. What would you do with $1 million to improve the value of your biobanks? The most important thing would be to implement data linkage programs to other data repositories such as the NSW Centre for Health Record Linkage program. DOI: 10.1089/bio.2011.9332

Royal Prince Alfred Collection Centre Site location: Royal Prince Alfred Hospital, Camperdown, Sydney Local contact: Prof. C. Soon Lee Date of operations (date of formal start of current bank operation): January 2007 Date range of cases: January 2007–present Example input statistics: Overall response to consent (Yes%/No%/Unknown %): 98% Yes Total individual cases held: 550 % cases associated with fresh-frozen tumor biospecimens: 48% % cases with paraffin embedded biospecimens: 80% % cases with blood and blood derivatives: 85% In your view, what are the most valuable/highest priority cases in your biobanks? Cases with full complement of frozen tissue, paraffin embedded tissue blocks, blood samples and accurately documented clinical annotation.

BIOBANK PROFILES 2) Employ 2 junior TBOs to look after tissue banking aspects. 3) Employ one senior TBO to look after all of the above. 4) Engage pathologists on a fee for service basis to look into some of the pathological aspects of the bank – e.g. review slides, mark slides for TMA construction etc. DOI: 10.1089/bio.2011.9333

Port Macquarie Collection Centre Site location: Port Macquarie Base Hospital Oncology Department, Port Macquarie, NSW Local contact: Dr. Stephen Begbie Date of operations (date of formal start of current bank operation): July 2007 Date range of cases: July 2007–present Example input statistics: Overall response to consent (Yes%/No%/Unknown %): 95% Yes Total individual cases held: 110 % cases associated with fresh-frozen tumor biospecimens: 0% % cases with paraffin embedded biospecimens: 90% % cases with blood and blood derivatives: 93% In your view, what are the most valuable/highest priority cases in your biobanks? Getting the collections done so that we can provide information for breast cancer research, performed by others (at this stage). What are your major challenges? Logistics What would you do with $1 million to improve the value of your biobanks? Provide additional breast cancer nurse hours to facilitate recruitment. DOI: 10.1089/bio.2011.9334

ACT Collection Centre Site location: The Canberra Hospital, Canberra, Australia Capital Territory Local contact: Prof. Desmond Yip/Prof. Jane Dahlstrom

What are your major challenges? 1) Accurate documentation of clinical information 2) Attracting reliable tumor bank officer (TBO) staff What would you do with $1 million to improve the value of your biobanks? 1) Establish a cell bank, construct tissue microarrays and cDNA arrays. Would need to employ 2 research assistants to look after these aspects of the biobank.

Date of operations (date of formal start of current bank operation): September 2009 Date range of cases: October 2009–present Example input statistics: Overall response to consent (Yes%/No%/Unknown %): 98% Yes
