Rhinology and skull base surgery: key updates in our field Nithin D. Adappa a and Samuel S. Becker b

The field of rhinology and skull base surgery continues to expand at a brisk pace. Accordingly, we have seen growth branching out to a number of different areas. One of the main challenges for us as clinicians is the ability to continue to identify the current literature and make both medical and surgical decisions based on current and newly identified literature. The goal of this issue of Current Opinion in Otolaryngology and Head and Neck Surgery is to review the latest literature on some commonly identified rhinologic medical topics, as well as the latest updates on a wide array of surgical procedures. We initially discuss contemporary literature on several topics that often come up in the office setting. Drs Dutta, Dubal and Eloy (pp. 2–7) explore the latest evidence between seasonal allergies, food allergies and rhinosinusitis. Drs Daramola and Becker (pp. 8–14) then give an update on the evaluation and treatment of olfactory disorders. We continue with in-office evaluation with a comprehensive discussion on the evaluation of rhinogenic headaches by Drs Sama and Agius (pp. 15–20). Finally, Drs Patadia and Welch (pp. 21–28) review the most recent literature on the role of immunotherapy in allergic fungal rhinosinusitis. We then shift into the latest discussion on commonly performed endoscopic sinonasal procedures beginning with Drs Ye and Zhou (pp. 29–33) updating us on current surgical management of inferior turbinate hypertrophy. Following this, Drs Thompson and Conley (pp. 34–38) review contemporary publications on a question that has been debated since the advent of endoscopic sinus surgery, the optimal size of a maxillary antrostomy. Drs Bury and Singh (pp. 39–48) similarly review the current literature on the role of nasal treatments on the management of snoring and obstructive sleep apnea. Finally, Drs Coughlan and Bhandarkar (pp. 49–52) delve into the newest publications and reviews on the utility of antibiotics in endoscopic sinus surgery. We complete this edition of the rhinology and skull base section with several literature updates on more complex and often less discussed topics. Drs Knisely, Harvey and Sacks (pp. 53–58) give us an update on endoscopic dacryocystorhinostomy and

long-term results following the procedure. Similarly, Drs DeConde, Suh and Ramakrishnan (pp. 59–64) will highlight the latest algorithms and treatment modalities for cerebrospinal fluid (CSF) rhinorrhoea. We then discuss a relatively new frontier in endoscopic skull base surgery with Drs Khalili, Palmer and Adappa (pp. 65–70) reviewing the recent literature on pediatric endoscopic skull base surgery. On a similar note, Drs Clavenna, Turner and Chandra (pp. 71–77) review endoscopic skull base reconstruction principles and the latest advancements with pedicle flaps. Finally, we end this rhinology section with an often-discussed topic in skull base surgery, the utility of lumbar drains. Drs Stokken, Recinos, Woodard and Sindwani (pp. 78–82) will review the recent literature on this topic. It is our goal, as well as the goal of our contributing authors, that this edition of Current Opinion in Otolaryngology and Head and Neck Surgery provides our readers with comprehensive up-to-date literature on a wide array of topics ranging from office-based evaluation and basic sinus surgery to advanced endoscopic skull base reviews.
