ACCEPTED MANUSCRIPT Letter to Editor, The pathophysiology of atrial fibrillation (AF) is complex and has not been
T
completely understood yet. However, current knowledge suggests that both electrical
IP
and structural remodelling of the atrium and the activation of inflammatory pathways may represent a prerequisite for AF (1,2). Total atrial conduction time (TACT) which can

be simply assessed by using tissue Doppler imaging (PA-TDI duration) and has been proposed as a marker of the extent of both electrical and structural remodelling of the

atria (3). Furthermore, epicardial fat, asserted as an endocrine active organ, is thougt to be an important source of proinflammatory mediators and given its direct neighboring to

heart potentially takes play in the pathophysiology of AF (4). Previous studies demonstrated that total or regional amount of epicardial fat has been associated with the new onset or recurrent AF (5,6). Therefore, it was the goal of this study to further

confirm the association of echocardiographically measured epicardial fat thickness (EFT)
TE
with the TACT in patients with lone paroxysmal AF.

This prospective study is consisted of 50 patients with lone paroxysmal AF and 50 healthy subjects. Patients who were at the age of ≥60 years, had pregnancy, valvular heart disease, connective tissue disease, renal dysfunction (serum creatinine levels ≥1.5

mg/dl), hepatic disorders, concomitant inflammatory, infectious or neoplastic diseases or any other systemic disorders including thyroid disorders were excluded from the study. The study was in compliance with the principles outlined in the Declaration of Helsinki and approved by Institutional Ethics Committee. Paroxysmal AF is defined as self-terminating episode, usually within 48 hours, may continue for up to 7 days (7). Lone AF was defined in patients who were
