Letter to the Editor Regarding screening for atypical femur fractures I greatly appreciate the comments of Drs Yueju and Yingze. It is clear that atypical femur fractures are rather rare outcomes. A very large sample size would indeed be required to attempt to screen for true atypical femur fractures. What is not known, however, is the incidence of pre-fracture lesions. Our study simply shows that, even in a high risk group of patients who have taken bisphosphonates for five years or more, radiographic precursors of atypical femur fracture were

not observed. While MRI or bone scan would be more sensitive, the cost of these imaging studies precludes their widespread use as a screening tool.
