A computed tomography (CT) scan revealed the presence of a 4.5 6.5 pseudoaneurysm of the noncoronary sinus of Valsava extending into the aortomitral fibrous trigone and the interatrial septum and compressing the

left atrial roof (Fig. 1). The aorta was nonaneurysmal and there was no aortic regurgitation. At the time of surgery, there was a pseudoaneurysm of the noncoronary sinus of Valsava, bulging posteriorly and compressing and adherent to the left atrium roof which was filled with thrombus (Fig. 2). The noncoronary sinus was replaced with a tailored Dacron patch (Fig. 2). The patient tolerated the procedure well and had an uncomplicated postoperative course.
