Primary Affective Disorders and HLA Antigens A. Mdjskf, P. Zvolskf and M. Dvofdkovd Institute of Hematology and Blood Transfusion, Prague and Psychiatric Clinic, Faculty of General Medicine, Charles University, Prague, Czechoslovakia In 125 patients with various types of primary affective disorders, 27 HLA antigens of the A, B and C loci were examined. Bw40 occurred more frequently in the primary form (20.83%against 7.64%in controls) and Cw4 in the unipolar form (25%)and, in disease in general, 16%,against 9.58% in controls. Following correction, however, these differences are statistically insignificant. Received f o r publication 9 September, accepted 7 October 1977

The question of HLA antigenic relationship t o primary (manic depressive) disorders has been dealt with by two groups of authors, each achieving different results. Strapiro et al. (1976) found an insignificantly higher frequency of HLA-A3 and B7 antigens in patients in general, but a statistically significant increase of Bw16 in the bipolar form. Stember & Fieve (1977) found an increased frequency of B5, B13 and Bw35 antigens; this result, after correction, is insignificant. Our short communication presents the results of studies on 27 HLA antigens 10 antigens of the A locus (A1,2,3,9,10, 11,28,29, Aw30/31,32), 14 antigens of the B locus (BS,7,8,12,13,14,18,27, Bw15,17, 21,22,35,40) and three antigens of the C locus (Cw2,3,4) in 125 patients with primary affective disorders. Seventyseven patients had bipolar (manic depression) form, 24 had unipolar (depression) form,

and 24 showed a combined form. Examination utilized the two-phase NIH microlymphocytotoxicity test. In contrast to the above-mentioned authors, we found no general or individual differences in the incidence of HLA antigens of the A or B loci, and for the uncorrected P, only Bw40 showed an increased frequency, in the unipolar form (uncorrected P = 0.03) (Table 1). Bw16 antigen was not determined. We found a higher incidence of Cw4 in the group as a whole (uncorrected P = 0.05), more so in the unipolar form (uncorrected P = 0.04). These results, however, were not significant after correction. Interestingly, Ivdnyi et al. (1977) also demonstrated an increased frequency of Cw4 in paranoid schizophrenia and, contrary to our results, their’s was significant. It remains to be elucidated whether antigens of the C locus, primarily Cw4,
