by Terry W. Trudeau, R.Ph. We are in the process of developing various clinical pharmacy programs in our hospital. Is there any means of reimbursement for these services once they are established?

ANSWER:

Reimbursement for clinical pharmacy services is an area currently being studied by many groups. To be sure, if the clinical pharmacy movement i s to grow and prosper, mechanisms must be established to adequately compensate for the labor costs usually associated with provision of such services. American Society of Hospital Pharmacists President Clifford Hynniman stated in his recent inaugural address, "There is no evidence that any third party (including the federal government) precludes reimbursement for clinical pharmacy services within the hospital setting." He commented that the problem is not lack of payment for what we do, but rather identification of specific programs provided and as a requisite for reimbursement, documentation of the program's positive contribution t o patient care. The Task Force on Payment for Pharmacy Services coordinated by the A.S.H.P. has indicated that billing for clinical services must be integrated with general hospital charges. Generally, if hospital administration i s convinced a specific service is needed, if there i s approval via some physician's committee (i.e., Pharmacy and Therapeutics, Patient Care, etc.) or direct physician order, and if third party insurers are provided the rationale behind the services should they request such, there will be no obstacle toward payment. The current literature describes many clinical services being reimbursed for both in hospital and ambulatory care patients.192J It is interesting to note the findings from a recent survey. When questioned about charges for clinical services, third party payers commented, "At least we will know what we are paying for.lr4 Fudge, R.
