Objectives.  The purpose of this study was to explore the microbiological and periodontal changes occurring in adolescents during 12 months of orthodontic therapy with removable aligners and with fixed appliances. Material and methods.  During the years 2012–2013, 50 teenagers aged 10–18 years with similar initial orthodontic conditions participated in this trial in a university clinic in northern Italy. After receiving professional oral hygiene and instructions on a standardized oral hygiene protocol, the adolescents were randomly assigned to either orthodontic treatment with traditional fixed brackets (n=25) or to treatment with Invisalign® aligners (n=25). Subgingival microbiological samples, probing depth (PD), plaque index (PI), and bleeding on probing (BOP) were obtained and documented from the mesiovestibular subgingival sulcus of the upper right first molar and left central incisor at the beginning of treatment and 3, 6, and 12 months later. Compliance with oral hygiene procedures, full mouth plaque score (FMPS), and full mouth bleeding score (FMBS) were assessed at the beginning of treatment and 12 months later. Two sample independent t-tests and the χ2 test were used to study whether the indices of periodontal health differed in the teenagers due to the experimental conditions. Studienziel.  Gegenstand dieser Untersuchung waren die mikrobiologischen und parodontalen Veränderungen bei Jugendlichen über 12 Behandlungsmonate mit herausnehmbaren Alignern oder festsitzenden Apparaturen. Methode.  Die Studie umfasst 50 Teenager (10–18 Jahre) mit vergleichbaren kieferorthopädischen Indikationen, behandelt in den Jahren 2012 und 2013 an einer norditalienischen Universitätsklinik. Nach einer professionellen Hygienesitzung und Unterweisung in einem standardisierten Mundhygieneverfahren erfolgte nach dem Zufallsprinzip die Zuordnung der Jugendlichen zu einer von 2 Behandlungsgruppen mit traditionellen Klebebrackets (n=25) oder Invisalign®-Schienen (n=25). Zu Behandlungsbeginn sowie 3, 6 und 12 Monate danach erfolgten mikrobiologische Probenentnahmen im Oberkiefer aus dem mesiovestibulären Sulkusbereich des rechten ersten Molaren und linken mittleren Schneidezahns sowie eine Beurteilung auf Sondiertiefen, Plaqueindex und provozierte Blutungen. Außerdem beurteilten wir zu Behandlungsbeginn und 12 Monate danach die Hygienedisziplin sowie die Gesamtindizes FMPS (Full Mouth Plaque Score) und FMBS (Full Mouth Bleeding Score). Zweistichproben-t-Tests für unabhängige Stichproben und der χ2-Test dienten dem Auffinden von Gruppenunterschieden bei den parodontalen Indizes. Resultate.  Tests auf 4 anaerobe Parodontalkeime waren für alle Patienten negativ. Die Invisalign®-Gruppe zeigte gegenüber der Bracket-Gruppe signifikant niedrigere Werte für Plaqueindex, Sondiertiefen, provozierte Blutungen sowie FMPS- und FMBSWerte. Auch die Hygienedisziplin war in der Invisalign®-Gruppe signifikant besser.

significantly lower and compliance with oral hygiene was significantly higher in the group treated with Invisalign® than in the group treated with fixed brackets. Conclusion.  Teenagers treated with removable appliances display better compliance with oral hygiene, less plaque, and fewer gingival inflammatory reactions than their peers with fixed appliances.

Introduction

Einleitung

Treatment with fixed orthodontic appliances increases the risk of white spot lesions, caries, and gingivitis during and after the orthodontic therapy. In particular, gingival tissue’s inflammatory reaction has often been reported, in conjunction with an increase in the plaque index, bleeding, and probing depth in association with teeth fitted with orthodontic brackets [3, 28]. The main factor for the increased accumulation of dental plaque and the inflammatory response seems to be the presence of retentive places around the components of fixed appliances attached to the teeth [8, 20]. Moreover, there is evidence of the significant loss of periodontal attachment after fixed orthodontic treatment [10]. Consequently, patients with fixed appliances should follow a very rigid oral hygiene protocol to prevent these side effects. Parallel to clinical investigations of periodontal indices, microbiological studies of the composition of subgingival dental plaque in patients with fixed orthodontic brackets reveal significant changes, suggesting that such treatment may stimulate the growth of a subgingival flora where some periodontopathogenic bacterial strains are prevalent, such as Porphyromonas gingivalis, Prevotella intermedia, Bacteroides forsythus, Actinobacillus actinomycetemcomitans, Fusobacterium nucleatum and Treponema denticola [18, 23]. Unlike fixed dental appliances, removable orthodontic appliances can be taken out and thus enable patients to practice oral hygiene procedures under ideal conditions [24, 26]. Invisalign® (Align Technology, Santa Clara, CA, USA) is a method for resolving orthodontic problems that exploits the action of a series of transparent removable aligners made by polymer material to gradually move the teeth in the planned position in a “virtual treatment”, using a proprietary software program. These aligners cover all the teeth plus the marginal aspect of the gingiva, and should ideally be worn 20–22 h per day and removed only for eating, drinking, and tooth brushing or flossing [5]. Being removable, Invisalign® has the potential of not hindering oral hygiene, since patients encounter none of the obstructions caused by brackets, bands, or archwires. Miethke et al. [15] showed that using Invisalign® led to an improvement in clinical periodontal status in adult patients compared to fixed lingual orthodontic devices, because of the absence of the biofilm typi-

Festsitzende kieferorthopädischen Apparaturen erhöhen das Risiko von Verfärbungen (“White-Spot-Läsionen”), Karies und Gingivitis während und nach der Behandlung. Insbesondere sind in Verbindung mit Klebebrackets an Zähnen viele Entzündungsreaktionen der Gingiva mit erhöhten Plaqueindizes, Blutungen und Sondiertiefen beschrieben [3, 28]. Offenbar sind es primär Retentionsnischen rund um die Komponenten der zahnbefestigten Apparatur, die solche Plaqueablagerungen und Entzündungsreaktionen fördern [8, 20]. Zudem existieren Belege für erhebliche Attachmentverluste nach festsitzenden kieferorthopädischen Behandlungen [10]. Patienten mit festsitzenden Apparaturen sollten mithin ein sehr strenges Mundhygieneregime einhalten, um solche Nebenwirkungen gar nicht erst entstehen zu lassen. Parallel zu klinischen Auswertungen parodontaler Indizes haben auch mikrobiologische Studien gezeigt, dass Patienten mit Brackets eine stark veränderte Zusammensetzung der subgingivalen Plaque zeigen und solche Behandlungen somit möglicherweise das Wachstum einer subgingivalen Mikroflora fördern, in der Parodontalkeime, wie etwa Porphyromonas gingivalis, Prevotella intermedia, Bacteroides forsythus, Actinobacillus actinomycetemcomitans, Fusobacterium nucleatum oder Treponema denticola, die vorherrschenden Bakterienstämme bilden [18, 23]. Anders als festsitzende Apparaturen können herausnehmbare Geräte eine optimale häusliche Mundhygiene ermöglichen [24, 26]. Die Invisalign®-Methode (Align Technology, Santa Clara, Kalifornien, USA) dient der Lösung kieferorthopädischer Probleme über eine Reihe herausnehmbarer durchsichtiger Kunststoffschienen, welche die Zähne (unterstützt durch eine “virtuelle Behandlung” in einer eigenen Softwareumgebung) Schritt für Schritt in ihre vorgesehene Position bringen. Diese Schienen überdecken alle Zähne samt der marginalen Gingiva, sollten im Idealfall 20–22 h täglich getragen und lediglich zum Essen/Trinken sowie zur häuslichen Mundhygiene mit Bürste oder Zahnseide herausgenommen werden [5]. Als herausnehmbares System bietet Invisalign® die Möglichkeit einer unbehinderten häuslichen Mundhygiene ohne Brackets, Bänder oder Führungsdrähte. Miethke et al. [15] zeigten, dass Invisalign® bei erwachsenen Patienten dank des Fehlens des für fest-

cally detected in patients wearing fixed appliances. Invisalign® treatment seems to offer several advantages in terms of maintaining oral hygiene [13]. No signs of gingivitis and improved gingival and plaque indices have been reported in different studies with patients undergoing orthodontic therapy with removable aligners [14, 15, 21]. Orthodontic therapy with fixed appliances has certain inherent drawbacks, including dental and soft-tissue discomfort, and poor aesthetics. Teenage patients may be particularly reluctant to undergo fixed-appliance treatment due to these drawbacks and for other social reasons [25]. Few studies in the literature take into account the effects of orthodontic therapy on the periodontal indices and subgingival flora in teenagers, whose compliance with oral hygiene often presents the clinician with a serious challenge. The aim of this preliminary clinical trial was to explore the microbiological and periodontal changes occurring in adolescent patients during the first 12 months of orthodontic therapy with fixed appliances and with the Invisalign® system.

Materials and methods The study included 50 teenagers (aged 10–18 years) of both sexes referred in 2012–2013 to the postgraduate clinic of the Department of Orthodontics, School of Dentistry, University of Insubria, Varese, Italy. The subjects fulfilled the following criteria for participation: non-smokers, good general and periodontal health, no extensive dental restorations or fixed partial dentures, lack of antibiotic therapy during the 3 months prior to the beginning of and during the study, and no use of antiplaque and oral antiseptic solutions during the trial. The orthodontic treatment plan did not include tooth extractions or other mechanics requiring the use of bands on molars. For each patient, a parent (or guardian) signed a written consent for the therapy and the clinical trial. Authorization by the Ethics Committee of the University of Insubria, Varese, Italy, was obtained. One month before the orthodontic therapy, professional oral hygiene was performed on all the patients who also received instructions on a standardized oral hygiene protocol. Using sealed envelopes (to “blind” the decision on the treatment to be performed), each teenager was randomly assigned to one of the two experimental groups: 25 patients were treated with Invisalign® aligners and 25 with fixed orthodontic appliances. The fixed orthodontic treatment was carried out with brackets bonded directly on incisors, canines, and premolars, and orthodontic tubes bonded directly on the first molars. Invisalign® aligners were given to patients with instructions to be worn 20–22 h per day and removed only for eating and tooth brushing. The patients had to wear each pair of aligners for 2 weeks before moving on to the next pair in the series. The health status of the periodontal tissue was determined by using periodontal indices and subgingival microbiological analysis at the beginning of the orthodontic therapy (T0), as well as 3 months (T3), 6 months (T6), and 12 months (T12) after the start of treatment. sitzende Apparaturen typischen Biofilms zu einem besseren Parodontalstatus als bei diesen führt. Invisalign®-Behandlungen scheinen im Hinblick auf die Weiterführung der häuslichen Mundhygiene in mehrerlei Hinsicht vorteilhaft zu sein [13], und verschiedene klinische Studien zu mit herausnehmbaren Alignern erbrachten keine Hinweise auf Gingivitis sowie bessere Gingival- und Plaqueindizes [14, 15, 21]. Kieferorthopädische Behandlungen mit festsitzenden Apparaturen haben bestimmte inhärente Nachteile wie unangenehme Empfindungen an Zähnen und Weichgewebe sowie ästhetische Mängel. Manch ein Jugendlicher sträubt sich aufgrund dieser Defizite gegen eine festsitzende Behandlung und aus anderen, im Bereich der sozialen Interaktionen angesiedelten Gründen [25]. In der Literatur finden sich bislang kaum Untersuchungen zu den Auswirkungen von kieferorthopädischen Behandlungen auf die parodontalen Indizes und die subgingivale Mikroflora bei Jugendlichen – also einer Altersgruppe, deren häusliche Mundhygiene nicht selten eine ernste Herausforderung für den Behandler darstellt. Gegenstand dieser klinischen Pilotstudie an Jugendlichen waren die mikrobiologischen und parodontalen Veränderungen in den ersten 12 Behandlungsmonaten mit einer festsitzenden Apparatur oder mit Invisalign®-Schienen.

Material und Methoden Die Studie umfasste 50 Jugendliche beiderlei Geschlechts im Alter von 10–18 Jahren, die 2012 und 2013 an die kieferorthopädische Klinik der Universität Insubria (Varese, Italien) überwiesen wurden. Als Voraussetzungen zur Studienteilnahme handelte es sich ausschließlich um Nichtraucher, die sich allgemein eine gute Gesundheit aufwiesen, ein gesundes Parodont, keine ausgedehnten Zahnfüllungen und keine Brücken, bis 3 Monate vor Studienbeginn keine Antibiotika eingenommen hatten und dies (nebst Verzicht auf plaquehemmende/antiseptische Mundspüllösungen) auch während der Studie nicht taten. Die kieferorthopädische Therapieplanung sah keine Zahnextraktionen und keine Mechaniken mit Bebänderung von Molaren vor. Für die Behandlung und die Studienteilnahme unterzeichnete jeweils ein Elternteil (oder Erziehungsberechtigter eine Einwilligungserklärung. Eine Genehmigung der Studie durch die Ethikkommission der Universität Insubria (Varese, Italien) liegt vor. Einen Monat vor der kieferorthopädischen Behandlung erhielten alle Patienten eine professionelle Mundhygiene sowie Anweisungen für ein standardisiertes häusliches Mundhygieneregime. Anhand von versiegelten Umschlägen (verblindete Entscheidung über den Therapiemodus) wurden alle Jugendlichen nach dem Zufallsprinzip einer von 2 Versuchsgruppen zugeordnet: 25 Behandlungen wurden mit Invisalign®-Schienen durchgeführt und 25 mit einer festsitzenden Apparatur, letztere per Direktbefestigung von Brackets an Schneidezähnen, Eckzähnen und Prämolaren sowie von Röhrchen an den ersten Molaren. Die Aushändigung der Invisalign®-Schienen an die Patienten war mit der Anweisung verbunden, dass sie 20–22 h täglich zu

the same operator at each visit, strictly respecting international criteria for determining periodontal index scores [22]. The full mouth plaque score (FMPS) as the percentage of dental surfaces with detectable plaque and the full mouth bleeding score (FMBS) as the percentage of sites with bleeding on probing were recorded at six sites of all teeth at T0 and T12. The subgingival microbiological samples, probing depth (PD), plaque index (PI) [12], and bleeding on probing (BOP) were recorded from the mesiovestibular subgingival sulcus of the upper right first molar and upper left central incisor [16] at T0, T3, T6, and T12. Probing depth (PD) was measured to the nearest millimeter on the periodontal probe’s scale (GoldmanFox, Hu-Friedy Mfg Co., Inc., Chicago, IL, USA). BOP was registered 20 s after probing (absent =0, present =1) [27]. PI was assessed according to plaque accumulation in the gingival area in four grades: 0 (no plaque/debris on inspection and probing), 1 (thin film of plaque only visible after probing), 2 (ribbon-like layer of plaque covering the sulcus and gingival crown areas but not filling interdental space), and 3 (thick layer of plaque already visible at inspection and filling interdental space). Oral hygiene compliance was assessed in each treatment group at the beginning of the study (T0) and at the end of the treatment (T12) by interviewing the patients about the time spent for home oral-care procedures according to the following scoring criteria: 0 (patients showing no improvement in oral hygiene compliance), 1 (patients showing slightly better oral hygiene compliance), and 2 (patients showing significant improvement in oral hygiene compliance). To standardize the procedure, the compliance evaluations were done by the same operator who had initially instructed the teenagers on the oral hygiene procedures and who collected the periodontal indices. The microbiological samples were collected right after recording PI but prior to the other clinical procedures. Following microbiological sampling, probing depth was measured and bleeding on probing registered. The subjects’ microbiological status was determined by investigating the presence or absence of four periodontopathic anaerobes (Prevotella intermedia, Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia). These samples were collected in dry field conditions by inserting one sterile paper point into the deepest part of the gingival sulcus for 30 s. After that, the paper points were put in a test tube, refrigerated at −20°C, and sent to the Department of Microbiology (DNA Sequencing Service), University of Cagliari, Italy, where the microbiological analyses were performed. Periodontal pathogens were detected by real-time PCR procedures [4, 19]. The molecular protocols used in this study have been previously described [6, 17]. Each paper point was suspended in 50 μl of pure dimethyl sulfoxide (DMSO) and centrifuged for 30 s; 2 µl of this was used as DNA suspension for realtime PCR reactions. The periodontal bacteria was quantified using the oligonucleotides described for conventional PCR. Real-time PCR was conducted using a LightCycler® instrument and a LightCycler® DNA Master SYBR Green I kit (Roche Diagnostics, Mannheim, Germany) according to the manufacturer’s

tragen und lediglich zum Essen/Trinken und Putzen herauszunehmen sein würden. Jedes Schienenpaar war 2 Wochen lang zu tragen und wurde anschließend vom nächsten Paar abgelöst. Die Bestimmung des parodontalen Gesundheitsstatus erfolgte über entsprechende Indizes und eine Auswertung der subgingivalen Mikroflora, jeweils durchgeführt zu Behandlungsbeginn (T0) sowie 3, 6 und 12 Monate (T3, T6, T12) danach. Alle klinischen Messungen bei den einzelnen Besuchen wurden unter gewissenhafter Befolgung der internationalen Kriterien zur Bestimmung parodontaler Indexwerte [22] stets von derselben Person vorgenommen. Hinzu kamen zu den Zeitpunkten T0 und T12 Totalbestimmungen des Anteils an Zahnstellen mit nachweisbarer Plaque (Full Mouth Plaque Score, FMPS) sowie des Anteil an Stellen mit Blutungen auf Sondieren (Full Mouth Bleeding Score, FMPS), jeweils beruhend auf 6 Stellen pro Zahn. Die mikrobiologischen Probenentnahmen sowie Ermittlungen der Sondiertiefen, des Plaqueindex [12] und der provozierten Blutungen erfolgten zu den Zeitpunkten T0, T3, T6 und T12 im Oberkiefer aus dem mesiovestibulären Sulkus des rechten ersten Molaren und linken mittleren Schneidezahns [16]. Die Taschentiefen wurden dabei auf den nächsten Skalenmillimeter der Parodontalsonde (Goldman-Fox, Hu-Friedy, Chicago, IL, USA) genau gemessen und die provozierten Blutungen 20 s nach Sondieren als 0 (nicht zutreffend) oder 1 (zutreffend) gewertet [27]. Beim Plaqueindex unterschieden wir je nach den Ablagerungen im Bereich der Gingiva zwischen 0 (keine Plaque/ Fragmente auf Begutachtung und Sondieren), 1 (dünner Film, erst nach Sondieren sichtbar), 2 (den Sulkus und die gingivalen Kronenbereiche bedeckender Streifen ohne Beanspruchung von Interdentalraum) und 3 (dicke, sichtbare und Interdentalraum beanspruchende Lage). Zur Beurteilung, wie gut die Patienten in beiden Gruppen die Hygieneanweisungen befolgten, wurden sie zu Studienbeginn (T0) und am Ende der Behandlung (T12) über die aufgewendete Zeit für die häusliche Hygiene befragt. Die Bewertung erfolgte als 0 (Patient befolgte die Anweisungen nicht besser), 1 (etwas besser) oder 2 (deutlich besser) und zwecks Standardisierung durch dieselbe Person, die bereits die primären Hygieneinstruktionen gegeben und auch die parodontalen Indizes erhoben hatte. Die mikrobiologische Probenentnahme wurde sofort nach Erhebung des Plaqueindex, aber noch vor Ermittlung der Sondiertiefen und der Blutungen vorgenommen. Zu Aussagen über den mikrobiologischen Status der Jugendlichen gelangten wir durch Austesten der Proben auf vier anaerobe Parodontalkeime (Prevotella intermedia, Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia). Die Probenentnahme erfolgte nach lokaler Trockenlegung durch Einführen einer sterilen Papierspitze in den tiefsten Abschnitt des Sulkus auf 30 s. Dann wurden die Papierspitzen in Teströhrchen auf –20°C gekühlt und für die Analysen an das Institut für Mikrobiologie (DNA-Sequenzierdienst) der Universität Cagliari versandt. Der Keimnachweis erfolgte mit Techniken der Polymerasekettenreaktion (PCR) im Echtzeitverfahren [4, 19]. Die moleku-

instructions. Tenfold serial dilutions of each bacterial strain in DMSO ranging from 107 to 102 cells/ml was prepared. These suspensions served as a standard curve for measuring the pathogen concentration. PCR mixture contained (20 µl final volume): 4 mmol MgCl2, 1 µmol of each primer, and 2 µl of DMSO suspension. The PCR program entailed (1) denaturation at 95°C for 30 s, (2) 40 cycles of 0 s at 95°C,10 s at 50°C, 12 s at 72°C, and (3) melting curve done for 0 s at 95°C, 45°C, 95°C. Transition rates were 5°C/s in the 72°C segment, 0.1°C/s in the 45°C segment, and 20°C/s for another step. Fluorescence was detected at the end of the 72°C segment in the PCR step (single mode), and at the 45°C segment in the melting step (continuous mode) in the F1 channel. During initial optimization of the real-time reaction, PCR products were analyzed using agarose gel and via a melting curve analysis to ensure correct sample product size. The positive reactions revealed 7–90°C Tm peaks. The amount of bacterial DNA in the samples was calculated according to the sequent formula [C = q*25], where C is the final bacterial concentration (total or single pathogen) in the specimen, where q is the bacterial number calculated interpolating the threshold cycle with a qPCR standard curve. Descriptive statistical analyses were compared to summarize the patients’ main characteristics in the two experimental conditions. Two sample independent t-tests were used to study whether the subjects’ full mouth plaque and bleeding score means differed according to the experimental condition at the study’s beginning and end.
