Hayden, M.D. Palmar arteriography in patients with acronecrosis complicating collagen vascular disease demonstrated multiple occlusions and stenoses which were most severe in the proper digital arteries. Involvement was prevalent at points of manual stress, i.e., adjacent to metacarpal-phalangeal and interphalangeal joints. An additional observation was the presence of incomplete superficial and deep palmar arches in all instances, a finding which diametrically contrasts with anatomical studies purporting to demonstrate complete palmar arches in 80 % of the normal population. is a frequent harbinger or late accompanyment of the collagen vascular diseases. While this phenomenon is primarily a vasospastic process, histologic (3, 5, 6) and arteriographic studies (3, 7) have indicated organic obstruction of digital arteries in some instances. The severity and extent of

these obstructive lesions have not been consistently related to the apparent severity or duration of the particular systemic disease (3, 5-7). Moreover, some patients with rheumatoid arthritis (5-7) and scleroderma (3) have demonstrated significant stenoses or occlusions of digital arteries in the absence of Raynaud's phenomenon or
