LETTERS TO THE EDITOR CONGENITAL URINARY AND MYELODYSPLASIA

ANOMALIES

To the Editor: We read with interest the report by Kreder et al.’ on anomalies associated with myelodysplasia. We recently reported our experience with congenital urinary abnormalities associated with neural tube defects2 We found significant urinary abnormalities in 11 of 178 children (6%) with a neural tube defect. The pattern of renal malformations observed by Kreder et al.’ conforms to that observed by us and other authors.2-4 The most common renal abnormalities observed are those of fusion and migration (horseshoe kidney, pelvic kidney, crossed ectopia) followed by abnormalities of formation (atresia, dysplasia). We also observed congenital lower urinary tract abnormalities including cloacal anomaly and urethral atresia. 2 We recommend that all children with a neural tube defect have an ultrasound of the urinary tract performed to look for associated congenital urinary abnormalities. To the Editor: I read the article by S. Rowe, S. Siegel, and W. Benedict on Ophthalmologic Manifestations of Genitourinary Diseases, published in the June issue (vol. 39, pages 523-525, 1992) of UROLOGY.

I would like to add that one must also look for ophthalmopathy of Graves disease, since testis cancers are known to cause hyperthyroidism.1,2 Steigbigel et al.’ reported in 1964 on a male patient with hyperthyroidism associated with embryonal cell cancer of the testis and a high serum level of human chorionic gonadotropin (hCG) . Dorr2 reported on a case of hyperthyroidism in a patient with embryonal cell carcinoma who had elevated BhCG, which had thyrotropic activity sufficient to cause hyperthyroidism because BhCG is functionally similar to thyrotropin.
