A rare case of myxoma of soft tissue presenting in a 34-year-old woman

THE myxoma is a tumour found almost exclusively in the maxilla and mandible, and arising only occasionally in other bones. Very few examples of myxomas of the oral soft tissues have been recorded. Only one case of myxoma of the gingiva has been presented by Bernier (1960).

CASE REPORT The patient was a housewife, aged 34 years, who was referred for investigation of the small mass on the gingiva. Examination revealed a small mass on the palatal surface of the maxillary gingiva, between the left central and lateral incisors. It was a firm, whitish nodule, about 0.5 cm in diameter. One day following the admission, an excisional biopsy was made under local anaesthesia. Microscopical examination revealed that the tumour was basically monomorphous, with small hyperchromatic stellate cells surrounded and separated by abundant pale blue myxoid ground substance (Fig. I). The stellate nuclei were fairly uniform in size and shape, with spindly amphophilic cytoplasmic processes trailing off into the myxoid substance (Figs. 2 and 3). No mitotic activity was present. The mucosa was intact (Fig. 1). Four months after the operation there had been no recurrence of the lesion.

DISCUSSION The myxomatous turnours found in the jaws are described as benign slowly growing, locally invasive lesions. Clinically, they present as a palpable and often painless swelling. Myxoma of the gingiva gives an anatomical picture of epulis (Barros et al., 1969). In considering the differential diagnosis, clinical findings are very similar to peripheral fibroma or fibrous hyperplasia of the gingiva. No difference in sex incidence has been noted but it is stated that occurrence is most frequent in young adults (Zimmerman and Dahlin, 1958; Barros et al., x969). Radiographically, the myxoma of bone is a radiolucent lesion that often resembles other benign processes such as cysts, fibromas, ameloblastomas, giant

granulomas, and fibrous dysplasia. Recognition of the tumour depends upon its characteristic appearance at operation and the histological pattern. In the radiology of the soft tissue myxoma found nearby, the bone may show a localised superficial resorption. Treatment should be radical surgery. Excision is the acceptable method for the soft tissue tumours. Myxomatous tumours of the oral soft tissues are very rare. Only one case of myxoma of the gingiva has been presented (Bernier, 1960). Other cases were found in which myxomata occurred in the palatal soft tissues (Babbitt & PfeitFer, 1937; Salama & Hilmy, rgjr), the lower lip (Traiger & Lawson, 1969), and the cheek (Spengos & Schow, 1965).
