The lesions consisted of hypopigmented macules that coalesced into a linear plaque that involved only the left side of her body. Within this plaque were several pearly papules and an ulcerated area that clinically resembled basal cell carcinomas. Histologic features of the hypopigmented linear plaque included multiple and multifocal basaloid proliferations in the papillary and mid dermis. The isolated pearly papules showed the typical findings of basal cell carcinoma. (J AM ACAD DERMATOL 1992;27:316-9.)

Basaloid follicular h a m a r t o m a ( B F H ) is a rare, nonhereditary disorder with distinctive histopathologic changes. A characteristic finding is multifocal islands in the papillary dermis and branching cords of basaloid epithelial cells in continuity at various sites with the basal layer of t h e epidermis and the hair follicles. Clinically, B F H can assume three different forms: a generalized type associated with myasthenia gravis a n d diffuse alopecia, 1-3 a localized linear and unilateral type, 4-7 and a localized type that mimics a plaque of alopecia. 7 The first case of linear and unilateral B F H was reported by Carney 4 40 years ago under the n a m e

linear unilateral basal-cell newts with comedones. To the best of our knowledge fewer than 10 cases of this unilateral form have been reported in the English literature. CASE REPORT A 36-year-old white woman had a persistent, asymptomatic, linear, hypopigmented, bandlike plaque since puberty. During the past few years she had noticed the appearance of small and slowly growing lesions within the plaque. The patient's past medical history was unremarkable. No family members had basal cell carcinoma. Her three sons were in good health and had no skin lesions.
