A 75-year old man underwent transapical aortic valve-invalve implantation for structural deterioration of a previously implanted bioprosthesis. Preoperative logistic EuroSCORE I and EuroSCORE II were 17 and 7.5%, respectively. He was readmitted to our institution 2 months later for severe dyspnoea. A large false aneurysm of the left ventricle was diagnosed (Fig. 1,

Video 1) and a successful surgical reoperation (Fig. 2) was performed. Conflict of interest: Augusto D’Onofrio is a physician proctor for Edwards Lifesciences.
