A 64 year old woman presented with leg oedema and a painful pulseless lump in the left popliteal fossa. The patient was pyrexial (>39 C) and anaemic. She had undergone a medial approach prosthetic femoropopliteal bypass for a popliteal artery aneurysm 4 years previously. No histological analysis had been performed at the time. Computed tomography angiography confirmed femoropopliteal bypass patency (arrow, panel A) including a mixed solid-cystic mass which was excised. Histological analysis (haematoxylin and eosin stain showing irregular vascular channels, mitoses, atypia, panel B) revealed a high grade angiosarcoma (CD31þ, CD34þ factor VIIIþ).
