INTRAMEDULLARY TUBERCULOMA OF THORACIC SPINAL CORD IN AN APPARENTLY HEALTHY NIGERIAN: CASE REPORT INTRAMÉDULLAIRE TUBERCULOME DE THORACIQUE MOELLE DANS UN NIGERIAN DE APPAREMMENT SAIN: RAPPORT DE CAS 1

We report a case of thoracic intramedullary tuberculoma in an immunocompetent male Nigerian with negative screening for tuberculosis. He presented with 8 months history of back pain and 2 months history of progressive weakness in both lower limbs. Physical examination revealed a well-nourished man with spastic paraplegia. Chest radiograph and CT scan were normal but Magnetic resonance imaging (MRI) of the thoracolumbar region showed cord oedema and circumscribed intramedullary lesions at D12 and L1 levels with target sign. The patient was promptly prepared and had D12 and L1 laminectomy and posterior myelotomy with excision of the intramedullary lesion. Histology showed granulomatous lesion with central caseation in keeping with a tuberculoma. He was treated with a 4-drug antituberculous regimen with physiotherapy and he made complete neurological recovery 8 months post-operatively. Intramedullary tuberculoma should be considered in differential diagnosis of intramedullary tumors in this environment. Treatment with antituberculous drugs results in good outcome. Abstrait Intra-médullaire spinale est une forme rare de tuberculose causant des blessures de la moelle épinière dans cet environnement. Nous rapportons un cas de tuberculome intra-médullaire thoracique chez un homme immunocompétent du Nigéria avec un dépistage négatif pour la tuberculose. Il a eu 8 mois de maux de dos et 2 mois de faiblesse progressive dans les deux membres inférieurs. L'examen physique a révélé un homme bien nourri avec paraplégie spastique. La radiographie thoracique et la tomodensitométrie étaient normaux, mais l'imagerie par résonance magnétique (IRM) de la région thoraco montré cordon œdème et circonscrites lésions médullaires au D12 et L1 niveaux avec signe cible. Le patient s'est fait rapidement opéré et avait D12 et L1 laminectomie postérieure et myélotomie avec excision de la lésion médullaire. L'histologie a montré une lésion granulomatose avec caséification centrale en accord avec un tuberculome. Il a été traité d'un traitement antituberculeux 4-drogue avec la physiothérapie et il a fait la récupération neurologique complète en 8 mois postopératoire. Tuberculome intra-médullaire doit être considérée dans le diagnostic différentiel des tumeurs intra-médullaires dans les environs. Le traitement avec des médicaments antituberculeux mène à un bon résultat. Mots clés: Tuberculome spinale intra-médullaire, Patient immunocompétent, Bon résultat.

patients have evidence of active tuberculosis in others part of the body and the incidence tends to be higher in immunosuppressed patients. Often diagnosis is made after an incisional biopsy but in the absence of tuberculosis in other systems, the diagnosis is often missed. The thoracic spinal cord is the most affected area of the spine by intramedullary 1,3 tuberculoma . Magnetic resonance imaging features of the lesion vary depending on the stage of disease.1 Surgery and antituberculous therapy often result in complete neurological recovery1-4. We report a case of an immunocompetent patient with thoracic paraplegia secondary to intramedullary tuberculoma who was treated surgically along with antituberculous therapy and physiotherapy with complete neurological recovery.

Nous rapportons le cas d'un patient immunocompétent avec paraplégie thoracique secondaire à tuberculose intra-médullaire qui a été traitée chirurgicalement avec la thérapie antituberculeuse et de physiothérapie avec récupération neurologique complet. Rapport de cas A 45 ans, l'homme s'est présenté à notre service avec la lombalgie de huit mois de durée et de la faiblesse progressive dans les deux membres inférieurs de deux mois de durée. Il a également associé la dysfonction bisphinctérienne. Il n'avait pas de perte de poids ou d'autres symptômes constitutionnels. Il n'a pas d'antécédents de tuberculose ou de l'exposition à un patient atteint de tuberculose et aucun antécédent de traumatisme à l'arrière.

Case Report A 45-year-old man presented to our service with low back pain of eight months duration and progressive weakness in both lower limbs of two months duration. He also had associated bi-sphincteric dysfunction. He had no weight loss or any other constitutional symptoms. He had no previous history of tuberculosis or exposure to a patient with tuberculosis and no antecedent history of trauma to the back.

L'examen clinique a révélé un homme d'apparence saine ayant les fonctions mentales supérieures intactes et les nerfs crâniens normaux. Les réflexes en vrac musculaire, le ton, la puissance et tendineux étaient normaux dans les deux membres supérieurs. L'examen des membres inférieurs a révélé paraplégie complète (puissance 0/5) avec le muscle réduite en vrac, le ton et les réflexes. Le réflexe plantaire était extenseur bilatéralement. Il y avait dépréciation sensorielle jusqu'à L1 dermatome en dessous duquel il était allodynie tactile. Alors que le reste de l'examen clinique était normal.

Clinical examination revealed a healthy looking man with intact higher mental functions and normal cranial nerves. The muscle bulk, tone, power and deep tendon reflexes were normal in both upper limbs. Examination of the lower limbs revealed complete paraplegia (power 0/5) with reduced muscle bulk, tone and reflexes. The plantar reflex was extensor bilaterally. There was sensory impairment up to L1 dermatome below which there was tactile allodynia. Otherwise the rest of the clinical examination was normal.

the same region (Figure 1). Plain radiograph and computed tomographic scan of the chest were normal. Hematological investigations were normal with a negative serological screening for HIV 1 and 2. A diagnosis of an intramedullary tumour was made with a remote possibility of a tuberculoma. He underwent D12- L1 laminectomy and posterior myelotomy with excision of a wellcircumscribed grayish mass under general endotracheal anesthesia. Histopathology revealed a granulomatous lesion with a central area of caseation in keeping with a tuberculoma (Figure 2).

étaient normaux avec un dépistage sérologique négatif pour le VIH 1 et 2. Un diagnostic d'une tumeur intra-médullaire a été faite avec une possibilité lointaine d'une tuberculose. Il a subi une D12- L1 laminectomie postérieure et myélotomie avec excision d'une masse grisâtre bien circonscrite sous anesthésie générale endotrachéale. Histopathologie a révélé une lésion granulomatose avec une zone centrale de caséification en accord avec un tuberculose ( F i g u r e 2 ) . La phase intensive de médicaments antituberculeux lui a été administrée avec des doses une fois par jour de l'isoniazide (300 mg), la rifampicine (600 mg), le pyrazinamide (1500 mg) et l'éthambutol (800 mg) qu'il subissait pendant 2 mois consécutifs. Dans la phase de continuation, il avait seulement rifampicine et isoniazide aux mêmes doses p o u r 1 0 m o i s s u p p l é m e nta i re s . L e carbamazépine brut à une dose de 200 mg lui a été administré deux fois par jour pour l'allodynie qu'il avait. Après laminectomie et le l'administration des antituberculeux, son dysfonctionnement bisphinctérienne et allodynie sont guéris alors que la puissance des membres inférieurs est graduellement revenue à la normale. Au moment où il a été observé pour le suivi de 8 mois après la chirurgie et le début de l'administration des antituberculeux, il était entièrement ambulatoire et a depuis retourné à son travail et activités pré-morbides.

He was subsequently commenced on intensive phase of antituberculous drugs with once daily doses of isoniazid (300 mg), rifampicin (600 mg), pyrazinamide (1500 mg) and ethambutol (800 mg) which he had for 2 consecutive months. In the continuation phase, he had only rifampicin and isoniazid at the same doses for additional 10 months. He was also commenced on plain carbamazepine at a dose of 200 mg twice daily for the allodynia he had. After laminectomy and commencement of antituberculous drugs, his bi-sphincteric dysfunction and allodynia resolved while power in the lower limbs gradually returned to normal. By the time he was seen for follow up 8 months after surgery and commencement of antituberculous drugs, he was fully ambulant and has since returned to his job and premorbid activities.

Dans une série de 29 cas de tuberculose vertébrale soignés sur une période de 20 ans, Nassbaum et al ont rapporté que seulement 7% étaient dus à tuberculoma8 intra-médullaire. En dépit d'une forte incidence et la prévalence de la tuberculose, il y a manque d'information sur la tuberculose intra-médullaire chez les adultes en Afrique sub-saharienne.

In a series of 29 cases of spinal tuberculosis seen over a period of 20 years, Nassbaum et al reported that only 7% were due to intramedullary tuberculoma8. In spite of a high incidence and prevalence of tuberculosis, there is paucity of information on intramedullary tuberculosis in adults in sub-Saharan Africa.

Tuberculose intra-médullaire (IMT) se produit dans le groupe d'âge jeune avec environ un tiers d'entre eux n'ayant pas des symptômes de la tuberculose ailleurs 9,10,11; notre patiente était âgée de 45 ans et n'a eu aucune preuve de la tuberculose dans toute autre partie du corps. La région la plus courante affectée par tuberculose intra-médullaire spinale est la région thoracique, survenant dans environ 70% des cas3 comme ce fut le cas chez un patient index avait une lésion qui affecte le D12 et les régions L1 du cordon. La raison probable pour laquelle intramédullaire tuberculose vertébrale implique la moelle épinière thoracique principalement pourrait être parce que cette région reçoit environ 45% de l'ensemble de l'approvisionnement en sang à la cord12 vertébrale et il est connu que Mycobactrium la tuberculose est une obligatoire aérobie qui se développe le mieux dans une région où la pression partielle de l'oxygène est élevée. Intramedullary tuberculoma (IMT) occurs in the young age group with about one-thirds of them not having evidence of tuberculosis 9,10,11 elsewhere ; our patient was 45 years old and did not have any evidence of tuberculosis in any other part of the body. The commonest region affected by spinal intramedullary tuberculoma is the thoracic region, occurring in about 70% of cases3 as was the case in the index patient had a lesion that affects the D12 and L1 regions of the cord. The probable reason why spinal intramedullar y tuberculoma involves the thoracic spinal cord predominantly could be because this region receives about 45% of the entire blood supply 12 to the spinal cord and it is known that Mycobactrium tuberculosis is an obligate aerobe which thrives best in a region where the partial pressure of oxygen is high. Intramedullary tuberculoma frequently presents with symptoms and signs of subacute spinal cord compression including motor, sensory or autonomic manifestations; Brown-Sequard syndrome has also been reported13. MacDonnell et al14 reported spastic and flaccid lower extremity weakness, back pain, bowel and bladder dysfunction and paraesthesia as a clinical presentations, which are similar to presentations of the index case. It may be difficult to differentiate spinal intramedullary tuberculoma from other 94

intramedullary lesions such as primary and metastatic spinal tumours and chronic granulomatous diseases like sarcoidosis, brucellosis and histiocytosis. However, the characteristic findings on magnetic resonance imaging (MRI) include low intensity rings with or without central hyper-intensity on T2W images and low to iso-intense rings on T1W images. T2W spinal MRI of our patient showed the typical “target sign” demonstrating low signal center (caseous material) surrounded by high signal rim (peripheral infective granulation tissue) which is a valuable means of differentiating a tuberculoma from other intramedullary lesions1, 11,15,16.

Il peut être difficile de différencier la moelle intra-médullaire tuberculose d'autres lésions médullaires telles que les tumeurs vertébrales primaires et métastatiques et les maladies chroniques granulomatoses comme la sarcoïdose, la brucellose et l'histiocytome. Toutefois, les résultats caractéristiques sur l'imagerie par résonance magnétique (IRM) comprennent des anneaux de faible intensité avec ou sans centrale hyper-intensité sur les images T2W et basse à anneaux iso-intense en T1. T2W IRM vertébrale de notre patient a montré la typique "signe cible" démontrant centre de signal faible (matériel caséeuse) entouré par la jante de signal élevé (tissu de granulation infectieux périphérique) qui est un moyen précieux de différencier un tuberculose d'autres lesions1 intra-médullaire,11,15,16.

In the management of intramedullary tuberculoma, medical, surgical or combined approaches have been used with good results in different series although there is no consensus on the ideal treatment11, 12,13,16,17. Although some authors recommend only medical treatment, surgery is usually indicated when the diagnosis is uncertain, the lesion is large and is accompanied by rapid deterioration in neurological functions or when there is no response to chemotherapy18. In this case the diagnosis was in doubt preoperatively which necessitated microsurgical excision which histologically was consistent with granulomatous lesion with central caseation in keeping with tuberculoma. As a general principle in the treatment of CNS tuberculosis, drugs that penetrate the CNS such as isoniazid, rifampin, ethambutol and 16 pyrazinamide should be used . This combination is usually administered in the intensive phase of 2-3 months followed by a continuation phase for a period of 9-10 months with Rifampicin and INH. There are few controlled studies detailing the specific combination and duration of antituberculous chemotherapy for intramedullary spinal

Dans la gestion des intra-médullaire tuberculose, les approches médicales, chirurgicales ou combinées ont été utilisés avec de bons résultats dans différentes séries bien qu'il n'y ait pas de consensus sur le traitement idéal 11,12,13,16,17. Bien que certains auteurs ne recommandent que le traitement médical, la chirurgie est généralement indiquée lorsque le diagnostic est incertaine, la lésion est grande et est accompagnée d'une d é té r i o rat i o n ra p i d e d e s fo n c t i o n s neurologiques ou quand il n'y a pas de réponse à chimiothérapie18. Dans ce cas, le diagnostic était dans le doute préopératoire qui a nécessité une excision microchirurgicale qui était histologiquement compatible avec lésion granulomatose avec caséification centrale en accord avec tuberculose. En principe, dans le traitement de la tuberculose du SNC, les médicaments qui pénètrent dans le système nerveux central tels que l'isoniazide, rifampicine, éthambutol et pyrazinamide devrait être administré 1 6 . Cette combinaison est habituellement administrée dans la phase intensive de 2-3 mois, suivie d'une phase d'entretien pendant une période de 9-10 mois avec la rifampicine et INH. tuberculoma but treatment can be extended for up to 18 months depending on the clinical state and the response of the patient1, 11,19. Our patient made a good recovery as he had normal power in his lower limbs following 8 months of antituberculous medications after surgery.

Notre malade a fait une bonne récupération, comme il avait la puissance normale dans ses membres inférieurs après 8 mois d'administration de médicaments antituberculeux après la chirurgie.

Conclusion: Intramedullary tuberculoma should be considered in differential diagnosis of intramedullary tumors in this environment. Treatment with anti-tuberculous therapy results in good outcome.

Conclusion: Tuberculose intra-médullaire doit être considérée dans le diagnostic différentiel des tumeurs intra-médullaires dans cet environnement. Le traitement avec les résultats des thérapies antituberculeuses mène à un bon résultat.
