(713)-774-7611 n the era of abused and misused antibiotics, resistance transfer factors, and selective organisms, we at Sharpstown General Hospita1 have taken a serious look at what effective means can be used to keep our staff alert to these potential problems. We have initiated the following integrated committee programs to keep our physicians informed of current trends, and receive from them suggestions and solutions to a longstanding problem. With information brought to them by the Quality Assurance Department, the Infection Control Committee and the Utilization Review Committee the Medical Staff at the committee level makes recommendations regarding antibiotic misuse. They also keep a close watch on normal hospital flora and usual sensitivity patterns. All of our phy- c, sicians take an active interest in this program and do their part toward better, more effective patient care. Each month the Utilization Review Committee reviews the use and kinds of antibiotics given patients- not only those covered by Title 5, 18 and 19, but also the patients reviewed for random sampling on a disease or diagnosis basis. Patterns of care and patterns of antibiotic usage are reviewed concurrently on a monthly basis in this committee. Results are then reviewed by the utilization Review Committee and recommendations forwarded to other physician committees. Of special interest is the fact that the Medical Audit Committee also reviews these results and recommendations since the entire antibiotic review program began with a medical audit of antibiotic usage in March of 1976. All antibiotic monitoring is considered followup to this original audit. One year later, April, 1977 a repeat audit of antibiotic usage was performed. Antibiotic statistics from the audit were reported and discussed by Dr. Edward Septimus at our monthly Medical Education Committee Meeting.

Gathering statistics without meaningful action would have been a waste of our time. Education of both physicians and nurses was felt to be the most important factor in resolving the problems identified. Beginning with surveillance and with the help of the pharmacy and laboratory, I am now aware of what antibiotics are used throughout the hospital. Each month charts of patients receiving more than one antibiotic, those receiving an inappropriate antibiotic, and those on high risk antibiotics are taken to the Pharmacy and Therapeutics Committee on a concurrent basis. The Pharmacy and Therapeutics Committee reviews the charts and checks for such things as cultures, and whether or not the organism was sensitive to the drug used. All charts of patients receiving high risk antibiotics are reviewed monthly to determine if proper lab studies were done during the course of administration. Cost to the patient, the use of more than one antibiotic for the same purpose, and duration of use is also taken into account. After this review, results are written into the minutes, a copy of the results is sent to the Infection Control Committee. After discussion their recommendations are added. The Utilization Review and the Surgery Committee also receive the results and discuss them at monthly meetings as does the Executive Committee, the governing board of the hospital. Our Professional Services Audit Committee, consisting of all head nurses and all ancillary department representatives, performs "nursing audits." Following the completion of the medical antibiotic audit, they decided to include criteria in their audits which related to antibiotics as well,i.e., correct administration of antibiotics, development and/or notation of side affects, and any increase or decrease in signs and symptoms of infection. The Infection Control Committee instituted another means of getting information on not only

hospital infections, kinds and sources, b u t also on use of antibioi cs. Each patient admitted to the hospital has a sheet on his chart for the nurse or the physician to complete. It determines if the patient had an infection on admission-the source-if cultures were taken, if antibiotics were given and the purpose of antibiotic therapy, i.e., infection specifically or prophylactically. We now obtain these statistics on every one of our hospitalized patients. It has become obvious to us that no action should or can be instituted without good objective and valid statistics to back it up. It is our good fortune to have a knowledgeable physician/pathologist and Chairman of the Infection Control Committee and Infectious Disease Specialist on our staff. They provide us with a wealth of resource material as well as take an active part in continuing education for both the medical and nursing staff. We are especially proud of our hospital's progressiveness in this field and feel that our system works as well as any now available for use in antibiotic review, with full backing from our 22

administrator, and the enthusiasm of our medical staff. It is our goal to expand our present system and to continue to provide the excellence in patient care for which Sharptown General Hospital has long been known.
