Sir, the recent report on ‘haemophagocytic lymphohistiocytosis and dengue’ is very interesting.1 de Koninck et al. reported a case of dengue with haemophagocytic lymphohistiocytosis and noted that ‘as dengue is a rising pathogen in tropical import diseases, clinicians must be aware of its rare but serious complications’.1 In fact, dengue can have several uncommon manifestations and complications and this can make the diagnosismore difficult.2 ‘Haemophagocytic lymphohistiocytosis, is sporadically seen in clinical practice.3 Sharp et al. noted that ‘persistent fever, pancytopenia, hepatosplenomegaly, and increased serum ferritin’ is the main finding.3 The diagnosis of this condition, is usually difficult and requires bone marrow examination.2,3 The problem is common in a dengue case with multiple organ disorder.4 The important consideration is the possible concomitant condition disorder that can result in

haemophagocytic lymphohistiocytosis. Good examples are sepsis5 and some viral infections (such as West Nile virus infection3).
