Exacerbated Retractile Mesenteritis – Rare Case of Abdominal Pain

Retractile mesenteritis, mesenteric panniculitis and mesenteric lipodystrophy are conditions of sclerosing mesenteritis. One histopathologic finding is predominant at a given time. Fibrosis is predominant in retractile mesenteritis, chronic inflammation in mesenteric panniculitis and fat necrosis in mesenteric lipodystrophy (George V et al. RadioGraphics 2013; 33: 1053 – 1080). Apart from ischaemia of the mesenterium, the pathogenetic mechanisms of the disease under discussion include diseases of the rheumatic type, trauma, pancreatitis and tumour diseases. It may also be caused by previous abdominal surgery panniculitis (Sabaté JM et al. AJR Am J Roentgenol. 1999; 172: 625 – 629, Zissin R et al. Br J Radiol. 2006; 79: 37 – 43).

Case description !

A 58-year-old woman presented at our hospital with increasing abdominal pain, which had occurred for the first time six months previously. She reported about a weight loss of 11 kg. She didn´t have fever or nocturnal sweating. There was no history of tumour disease, surgery or trauma. Gastroscopy and colonoscopy already performed and yielded normal findings.

Diagnostics !

Laboratory tests produced a CRP value of 16.58 mg/dl (normal range 0 – 1.2 mg/dl), a leukocyte count of 13.34 103/µl (4 – 10 103/ µl) and a BSR of 94 mm after one hour and 99 mm after two hours (normal range 6 – 10 and 5 – 20 mm). The other laboratory values were normal. Ultrasound did not yield any findings pointing towards a diagnosis. A CT examination of the abdomen was performed with a 16-line multislice CT (Brilliance CT 16, Philips Healthcare, Hamburg) and a collimation of 16 × 1.5 mm, an FOV of 180 × 180, a matrix of 512 × 512 and a tube voltage of 120 kV at 375 mA after intravenous administration of contrast medium of 150 ml Imeron 350, and oral and rectal contrasting of the gastrointestinal tract with di-

operative day. The patient has remained free of symptoms to date and has been requested to return in one year for a followup MRI.

Sclerosing mesenteritis is an umbrella term for mesenteric panniculitis, mesenteric lipodystrophy and retractile mesenteritis and consists as an inflammatory process of unknown etiology that involes the adipose tissue of the mesentery. Sclerosing mesenteritis belongs to the group of IgG4-related sclerosing diseases (ISD) (George V et al. RadioGraphics 2013; 33: 1053 – 1080). Sabaté et al described two histologic types, the acute or subacute form known as mesenteric panniculitis and the chronic form known as sclerosing or retractile mesenteritis. (Sabaté JM et al. AJR Am J Roentgenol. 1999; 172: 625 – 629, Daskalogiannaki M et al. AJR Am J Roentgenol. 2000; 174: 427 – 431). Typical morphological findings in CT and or MRI are a well-defined fatty mass with higher attenuation than that of retroperitoneal or subcutaneous fat tissue and displacing neighboring intestinal loops, lymph nodes within this structure, a “fat ring sign” surrounding blood vessels, lymph nodes and tumoral pseudocapsule (Gögebakan Ö et al. Eur J Radiol. 82: 1853 – 1859.). The mesenterial lymph nodules < 5 mm are possible in connection with the inflammatory changes (Amor F et al. Int Arch Med. 2011; 4: 22, Daskalogiannaki et al. JBR-BTR 2011; 94: 241 – 246). Imaging by CT the “fat ring sign” around mesenteric vessels and nodules had attenuation as low as normal adipose tissue. The tumoral pseudocapsule impressed as a peripheral band with soft-tissue attenuation and separated normal from inflammatory mesentery (Sabaté JM et al. AJR Am J Roentgenol. 1999; 172: 625 – 629). The “fat rigng sign” and the tumoral pseudocapsule are associated with mesenteric panniculitis and mesenteric lipodystrophy. A mesenteric mass with higher attenuation is the most common imaging appearance of sclerosing mesenteritis. In the case of retractile mesenteritis the mesenteric mass is denser and more homogeneus than in the case of mesenteric panniculitis (George V et al. RadioGraphics 2013; 33: 1053 – 1080). The characteristic findings of MP in MRI are the high signal intensity in comparsion with mesenterial and subcutanues fat. Ghanem et al. described a hypointense capsule around a hyperintense lesion in the T2-

(Amor F et al. Int Arch Med. 2011; 4: 22). Sclerosing mesenteritis must be differentiated from Crohn´s disease, liposarcoma, carcinomatosis, a carcinoid tumour, a desmoid tumour and in particular a lymphoma. The lack of a “fat ring sign” and tumoral pseudocapsule is indicative. A paraneoplastic etiology of MP has been postulated (Daskalogiannaki M et al. JBRBTR 2011; 94: 241 – 246). Gögebakan Ö et al. refute the prevalent notion that MP is a paraneoplastic phenomenon (Gökebakan Ö et al. EJR 2013; 82: 1853 – 1859). Ultimately, a laparoscopic biopsy is to be regarded as the gold standard for diagnosis by exclusion (Sabaté JM et al. AJR Am J Roentgenol. 1999; 172: 625 – 629). Abdominelle Beschwerden im Kindesalter sind häufig. In bis zu 90 % der Fälle kann keine organische Ursache diagnostiziert werden, doch sind insbesondere virale Gastroenteritiden, Obstipationen und Harnwegsinfekte ätiologisch häufig (Spee et al. Scandinavian J of Prim Health Care 2013; 31: 197 – 202). Zu beachtende chirurgische Differenzialdiagnosen sind u. a. die akute Appendizitis, das akute Meckel-Divertikel, sowie die ileozökale Invagination. Im Rahmen der bildgebenden Diagnostik ist immer auch an eine abdominelle Raumforderung zu denken, wobei typische Tumoren des Kindesalters, wie Nephro- und Neuroblastome zunächst im Fokus stehen. Im Falle eines sonografisch detektierten Tumors bietet sich die Magnetresonanztomografie zur weiteren Charakterisierung und zum simultanen Erst-Staging an. Der nachfolgende Fall beschreibt den Verlauf eines 7 Jahre alten Jungen mit einem Nebennierenrindenkarzinom, einer sehr seltenen Tumorentität im Kindesalter.

Der 7-jährige, altersgerecht entwickelte Patient stellte sich mit plötzlich aufgetretenem Unwohlsein, Erbrechen, sowie einer palpablen Raumforderung des rechten Oberbauchs in der universitären Kinderpoliklinik vor. Die initiale Abdomensonografie zeigte eine echoinhomogene, mäßig perfundierte, retroperitoneale Raumforderung von 11 × 9 × 14,5 cm mit Kompression der Vena cava inferior und echoreicher kapselartiger Begrenzung. Die am selben Tag durchgeführte MRT bestätigte den rechts-retroperitonealen Tumor mit Ausdehnung in die rechte Nebennierenloge und Bezug zu Zwerchfell, Leber und rechter Niere ohne eindeutige Infiltration. Im Vergleich zur Leber wies die Raumforderung ein T1w-hypointenses und T2w-hyperintenses Signalverhalten auf. Insbesondere in den zentralen Anteilen zeigte sich ein starkes T2-Signal. In der diffusionsgewichteten Bildgebung war eine Diffusionsrestriktion in den peripheren Tumoranteilen, nicht aber in dessen Zentrum erkennbar. Nach i. v. Kontrastmittelgabe zeigte sich ein inhomogenes Enhancement der peripheren Tumoranteile nicht jedoch zentral, sodass die Bildbefunde in der Zusammenschau als große zentrale Tumornekrose interpretiert

wurden. Auch die in der Sonografie dargestellte kapselartige Begrenzung zeigte ein " Abb. 1). In der nachKM-Enhancement (● folgenden FDG-PET/CT zeigte der Tumor eine kräftige FDG-Aufnahme der peripheren Anteile bei gleichzeitig zentraler Stoffwechselinaktivität im Sinne der Nekrose. Ferner manifestierten sich in der CT streifige Dichteanhebungen, die als schollige " Abb. 2). Verkalkungen zu werten waren (● Letztlich konnte bei bereits großem Primarius eine Fernmetastasierung ausgeschlossen werden. Laborchemisch fiel eine mäßige Erhöhung der neuronenspezifischen Enolase (NSE) auf 90 ng/ml (< 15) auf, während die Plasmaspiegel von Dehydroepiandrosteronsulfat (DHEAS) mit 2329 µg/dl (< 119) und von Androstendion mit 9,8 µg/l (< 0,55) deutlich erhöht waren. Begleitend zeigte sich eine exzessive Ausscheidung von DHEA und seiner Metaboliten im 24h-Sammelurin. Die Zusammenschau der erhobenen Befunde lenkte den Verdacht auf einen stoffwechselaktiven Tumor der Nebennierenrinde. Nach sonografisch gesteuerter Stanzbiopsie bestätigte die histopathologische Begutachtung einen primär von der Nebennierenrinde ausgehenden Tumor, woraufhin bei cT3 cM0-Tumorsituation die kurative Resektion geplant wurde. 11 Tage nach Erstvorstellung erfolgte diese mittels thorakoabdomineller radikaler Adrenalektomie rechts unter Einbeziehung des Punktionskanals, mit systemischer Lymphadenektomie, sowie Zwerchfellteilresektion. Hierbei erfolgte ferner eine En-bloc-Nephrektomie um

weighted sequence, which strongly concentrated contrast medium in the gadolinium-enhanced T1 sequence (Ghanem N et al. J Magn Reson Imaging. 2005; 21: 632 – 636). Interestingly we found a hypointense signal in the T2 weighting which has already been discussed as a sign of fibrosis (Kronthal AJ et al. AJR AM J Roentgenol 1991; 156: 517 – 519). These mesenterial changes are mostly found within the context of an examination for other reasons. The clinical symptoms of sclerosing mesenteritis can be non specific in the acute stage. Beside the feeling of abdominal fullness, nausea, lethargy, and pain in the upper and middle abdomen, an unclearly definable mass can be palpated in the abdomen
