What Is Happening? This issue of What Is Happening slightly differs from the usual format as three transfusion related communications arrived at the same time and for the sake of unnecessary delay in publication I have pleaded to publish in the same issue of journal. The first paper is by Tor Hervig et al. from Bergen dealing with the role of autologous blood on postoperative infection in patients undergoing elective orthopaedic surgery. A randomised study carried out in Norway comparing allogeneic red cell transfusion with autologous unwashed salvaged blood showed low postoperative infection rates in both groups. Low preoperative haemoglobin concentration was a significant risk factor for need of transfusion. Larger clinical randomised trials are needed to investigate if stimulation of the immune system by autologous salvaged blood protects against postoperative infections.

The second paper is also from Bergen dealing with change in guideline to prevent/reduce the risk of bacterial contamination. The third article by Thierry Bourneuf and close colleagues deals with recent advances in multi-faceted regenerative lives of expired platelets, an enjoyable reading for all interested readers in transfusion science. I am grateful to Gail Rock for agreeing with this move for the sake of rapid publication as the essential aim of What is Happening?
