Impacted canines, particularly in the maxilla, are frequently encountered in orthodontic practice. Unfortunately, depending on their position, correction can often be difficult and sometimes unsuccessful. Thorough diagnosis along with orthodontic treatment combining bone-supported anchorage and cantilever springs appears, in our view, to offer the best solution to this tricky orthodontic challenge while limiting, as far as possible, the risk of failure. Introduction

Impacted canines rank as the third most common form of dental impaction, with an incidence of 2 to 4.3%, according to the study [1,2]. Impaction is palatal in 50% of cases [1] (80% for other authors [3]), buccal in 30% and intermediary in 20% of cases. This anomaly is ten times more common in Caucasians as compared with Mongoloid populations, while the difference based on gender shows a very slightly higher prevalence among women.

Early diagnosis [8,9] enables adoption of frequently effective preventive procedures [10–13]. Late diagnosis generally entails initiation of orthodontic protocols presenting certain drawbacks which can result in treatment failure. According to Becker et al. [14], the most common causes are, in order of frequency, inadequate means of traction (48.5%), improper localization of the point of traction on the impacted tooth or inappropriate direction of traction (40.5%), ankylosis (32.4%), which means, in 89.1% of cases, a practitionerrelated cause! Treatment can only be successful if the orthodontic therapy used is coherent and enables the practitioner to manage both the anchorage and the direction of traction. So, what is the answer?

The etiology of maxillary canine impaction is multifactorial. A non-exhaustive etiological analysis reveals causes of a general (recessive genetic disorders) or local nature (obstacle on the eruption path) [3,4]. Impacted canines are occasionally associated with agenesis of 12 and 22 [5], with anomalous morphology of the lateral incisors [6] or with a narrow maxilla [7].

Traditionally, anchorage devices are tooth-borne. During disimpaction, reaction-type movements can be observed at the teeth on the arch being treated. When disimpaction is performed by means of a fixed buccal appliance and traction supplied by an elastic, the extrusion of the impacted maxillary canine will inevitably, by reaction, trigger intrusion of the anchoring teeth or group of teeth (fig. 3). canine will generate a contrary reaction movement in a palatal direction of the teeth adjacent to the canine. These side-effect movements are frequently undesirable. Intramaxillary devices such as transpalatal arches are useful ways of controlling certain types of unwanted movement but provide no anchorage. The most reliable means of anchorage in DFO are temporary anchorage devices (TAD) [18]. TADs must be used when the conventional mechanical disimpaction system is inadequate [19]. TADs are also indicated when, for various reasons, the orthodontic appliance has to be reduced to a minimum. But how can continuous forces be achieved?

In orthodontics, a cantilever spring is a mechanical system made from a length of archwire (fig. 4). In cases involving canine disimpaction, the fixed anchorage point is located posteriorly. The free anterior end provides a lever arm attached to the tooth to be disimpacted. The preactivated cantilever arm provides control over the orientation, direction and intensity of the force [20,21]. The mechanical action of the cantilever spring is similar to the deformation of a beam set in a wall and subjected to a force applied at its extremity (fig. 4). The segment of archwire used to make the spring is rectangular to ensure that play is minimal when it is inserted in a similarly rectangular slot. This set-up makes it easier to control the orientation of the force and to limit any undesirable movements of the spring and thus enhance patient comfort. The force “F” released by a cantilever spring is dependent upon the length “L” of the lever arm, the elasticity modulus of the alloy “E”, the moment of inertia “I” (depending on the cross-section of the archwire) and its preactivation “D” (fig. 4). For a given preactivation, the moment “M” of the force “F” which is developed is equal to the length of the arm lever “L” multiplied by the intensity of the force, irrespective of where the point is situated on the lever arm. As the load/deflection ratio remains constant, the longer the lever arm, the lower the force. During tooth movement, the slight disactivation of the lever arm allows delivery of an almost constant force (fig. 4).

— it is the only mechanical system which enables the orientation of the force to be selected irrespective of the buccal or palatal position of the tooth; — it provides a force which is as constant as possible using a low load-deflection ratio [15]; — the load-deflection ratio is modulated: by the position of the cantilever’s support points which varies the length of the lever arm. The more posterior the support point, the longer the lever arm and less is the intensity of the delivered force; by using an alloy with an elasticity modulus suited to the length of the lever arm. A long lever arm allows the use of a less elastic wire. In contrast, a short lever arm immediately requires an alloy with a low elasticity modulus (molybdenum-titanium, nickel-titanium) or the addition of loops to reduce the loaddeflection ratio.

Optimum mechanical and physiological efficacy can be achieved during disimpaction of a maxillary canine if the following specifications are observed: — use of a consistent system whatever the position of the canine (buccal or palatal traction) delivering continuous forces in the 3 planes of space. The cantilever spring’s mechanical system combines continuous forces and traction in the 3 planes. The amplitude required to obtain low-intensity forces is achieved by ensuring sufficient distance between the anchorage point of the system and the point of traction of the impacted tooth and by accurately selecting the characteristics of the wire; — a system which is autonomous and, whenever possible, independent of the other teeth to avoid unwanted movements. TAD in the shape of orthodontic plates or screws offer the most appropriate solution. Technically speaking, the buccal position of TAD, as opposed to the palatal, is easier to achieve and offers a wider choice for the orientation of the traction [22]. TAD ensure consistency of the mechanical system; — good cantilever stability at the fixation points to limit mucosal injury and avoid occlusal interference. Stability is provided by a double attachment point (cf. Section Anchorage screws).

(fig. 5). To this end, bracket-head miniscrews (Ancotek.Tekka System) are used. In order to reduce wire play in the screw head, the crosssection of the cantilever should match as closely as possible the dimensions of the slot. However, the length of the slot is insufficient to stabilize the cantilever in all 3 dimensions of space. Hence, a second support point is necessary. This can be either the multi-attachment appliance (fig. 6) if there is one, or a second miniscrew if there is no fixed appliance (fig. 7). Addition of a second support point eliminates clockwise and counter-clockwise effects on the miniscrew during activation and deactivation as these movements can loosen the screw. It also allows better control over the movements of the free section and thus avoids injury to the mucosa and occlusal interference. The cantilever is fabricated in situ respecting clinical parameters (length of the lever arm, shape and position of the tooth, elasticity modulus of the alloy).

A 13-year-old girl presenting with an asymmetrical Class III requiring orthodontic-surgical treatment (figs. 11–13). This treatment was scheduled at end of puberty, i.e. around 16 years of age. At the first appointment, 53 was in place. The dental panorex showed the right maxillary canine in a high buccal position surrounded by supernumerary teeth which could jeopardize disimpaction treatment (possible ankylosis).

The advantage of this system was to reposition the impacted tooth without initiating the multi-attachment phase, which was postponed until the second presurgical orthodontic preparation stage. If the canine turned out to be ankylotic, the treatment plan could be modified with no loss of time. Furthermore, as the mechanical system was consistent, there would be no parasitic movements related to the ankylosis. With this protocol it is possible to make a differential diagnosis. After extraction of the supernumerary teeth and peroperative bonding of the impacted 13, traction was applied to the tooth by a cantilever using a .021  .021” TMA wire. At 1 month, the system was checked and adjusted and further 6-weekly appointments were scheduled. The form of the archwire was adapted whenever necessary at these appointments, depending on the movements desired. After 6 months, the canine was repositioned as optimally as possible in the existing space. A control X-ray was taken to check the status of both periodontium and bone and the absence of apparent lesion.

A 15-year-old young man at first consultation with an appliance placed prior to consultation at our office (figs. 14–17). In addition to a full Class II, 13 and 23 were observed to be palatally-impacted. The roots of 11.21 appear to be short and bent distally. There was therefore a real risk of root resorption. Treatment needed to be as simple as possible and to make minimum demands on the maxillary complex in order to avoid any iatrogenic effect. We replaced the initial quad-helix with a transpalatal arch to provide posterior support for the disimpaction cantilevers. Two miniscrews were placed between the first and second right and left maxillary premolars. The cantilevers made from .021  .021” TMA wire were attached to both the miniscrews and the buccal molar tubes. Molar tubes ensure cantilever stability while miniscrews provide anchorage and permit loading of the system. The latter are mechanically coherent (minimum demands on natural anchorage) and allow impacted

canines to be drawn distally and vertically in a first stage, and then buccally (fig. 2). Following an appointment at 1 month, then regular 6-weekly visits (adjustment of the direction of traction and verification of traction intensity), 13.23 were positioned in their anatomic spaces. A full maxillary multibracket appliance then replaced the cantilevers to bring 13.23 to their final position. Five months later, the canines were in place. The multi-bracket appliance was then removed. Final correction of the Class II will be performed using an orthodonto-surgical protocol on completion of growth. In all, the overall procedure to reposition 13.23 lasted 11 months. The control X-ray showed the periodontal and bone status to be correct with no apparent lesion. No iatrogenic effect could be observed at the maxillary incisor roots, which remained short.

Technically speaking, prior opening of the space at 23 was mandatory, requiring Class II correction by molar retraction as well as correction of the overbite by incisor intrusion. Repositioning of 23 will be done at a later stage. If 23 were to be ankylotic – a major risk with adult patients – an implant will be inserted to replace the natural tooth, but in an improved occlusal setting. To preclude any unwanted movements due to possible ankylosis, the mechanical disimpaction system must be kept separate from the rest of the appliance. A cantilever spring is then chosen for fixation to the orthodontic anchorage plates in order to ensure prior molar retraction and intrusion of the maxillary incisors. The maxillary multi-bracket appliance was placed first. “Bayonet Cantilever” intrusion archwires were used to tackle the vertical problem. Maxillary retraction systems connected to the plates were used during the same phase in order to achieve a bilateral Class I. The mandibular arch was fitted at a second stage (following correction of the maxillary overbite) until Class I occlusal balance was achieved. Exposure and bonding of the impacted 23 were then performed. A new disimpaction cantilever arm was then put in place. As no ankylosis occurred, the tooth was easy to guide in the 3 spatial dimensions to its physiologic space in the course of a first appointment at 1 month followed by 6-weekly visits

Discussion

When making a diagnosis and drawing up the treatment plan, the practitioner should first and foremost bear in mind the architecture of the periodontium. When the canine erupts into the oral cavity, fusion occurs between the adamantine and the gingival epithelia. The quality of the periodontium surrounding the canine will depend, therefore, upon the exact location where it erupts. In cases involving a buccal canine, the practitioner must therefore ensure that the periodontium which will receive the canine and accompany its development is of sufficient quality. A translational flap is recommended in the event of the tooth erupting outside keratinized tissue or in tissue with inadequate available thickness. In the case of palatal canines, a window can be created at flap level in order to reduce the risk of eruption being impeded by a thick layer of fibromucosal tissue [2]. During the treatment phase, the mechanical system must be effective, whatever the position of the impacted canine. Hence, in our view, the cantilever system offers the optimum solution. Whether the tooth is vestibular, palatal or mediallypositioned, a cantilever can be readily fashioned, modified and adjusted in accordance with the evolving clinical situation, in terms of both intensity and direction. Adjusting this system requires little or even less chair time than adjusting overlay loop archwires and provides, in our view, a more reliable and predictable result. Similarly, in our opinion, the buccal position of the cantilever system offers a more dependable solution, even for a palatal canine. Indeed, thanks to the length of the wire used (increased distance between the fixed point and the point of application of the force) and to the flexibility of the wire, its range of action is optimized, thus guaranteeing control over the direction and intensity of the force. The increased length of the active wire allows effective control over the orientation of traction in the XX’, YY’, ZZ’ axes, whether independently or in combination. Finally, it should be borne in mind that interference from palatally-positioned mechanical systems can trigger lingual parafunctions, leading on to failure of the treatment due to the loss of miniscrews [23]. Whatever the position of a mechanical system, system stability is a primordial criterion. The stability of a cantilever-TAD system can only be optimal if there are two anchorage points. A single point on a single miniscrew, apart from its lack of

lateral stability giving rise to lesions at the cheeks and/or mucosal tissues, will trigger rotation torque at the miniscrew and increase its instability. Two anchorage points, using miniscrews for example, check the rotation torque and avoid loss of the miniscrew. This dual anchorage can also be mixed (transpalatal arch-miniscrew) in order to reduce the number of miniscrews required. Natural anchorage will be called into play in this setting, offering an acceptable though theoretically less than ideal situation. Regarding the anchorage plates, torque is autocontrolled thanks to the two, or even three, anchorage points on the body. There remains the problem of lateral instability due to the existence of a single anchorage point on the plate, a situation which is all the more critical when the free segment is long. The use of a clip-on hook on the main archwire of the multibracket appliance controls lateral movements. The junction is free with a single contact point. The point of application of the force generated during loading of the system is located at a distance from the clip-on hook point and draws on natural anchorage, creating a moment which could theoretically trigger radiculo-palatal torque at the main archwire. In this situation, the multi-bracket archwire used is a .017  .025” SS slotted into .018  .025” brackets. To date, no undesirable movement has been observed clinically during usage of this system. DFO treatment inevitably involves a compromise between the ideal outcome and what is technically possible. In these three cases, the classical technical resources were inadequate and uncertain. We are convinced that the use of the TAD-cantilever tandem enabled us to achieve optimal results and easily, for both practitioners and patients.
