A 70-year-old man underwent an aortic valve replacement for calcified aortic stenosis. Preoperative coronarography showed normal coronary arteries (Fig. 1). Ventricular fibrillation occurred after the sternum closure and emergency coronary angiography (Fig. 2) showed a
subocclusion of the right and the left anterior descending coronary arteries probably due to calcified emboli from the annulus. The balloon angioplasty was unsuccessful. The patient died of refractory cardiac arrest before surgical revascularization could be performed.
