A 17-year-old boy with symptoms of dyspnea in New York Heart Association class III was admitted with a diagnosis of coarctation of the aorta distal to the left subclavian artery. An aortogram showed a dilated ascending aorta (6.5 cm) and a well-developed collateral supply to the descending aorta through the internal mammary arteries (Figure 1). The patient underwent ascending aortic replacement with a 24-mm Dacron graft and an ascending aorta-to-descending aorta interposition graft, via a sternotomy. An 18-mm graft was sutured to the descending aorta and the proximal end of this graft was anastomosed to the ascending

aortic graft. The postoperative aortogram showed a patent graft with no contrast leak (Figure 2). The patient was discharged in a stable condition on the 8th postoperative day.
