La patholog ic th yro't'di enne sc rcnco ntrc quatre a cinq fois plus frequ cmmcnt chez la femme qu e chez l'hommc , en parti culicr au cours de la periodc «de fcrtilite». II n 'est en co nsequence pas inh abi tucl de decouvrir des anomali es bi oc himi ques th yro't'di ennes chcz la femme encc int c ~t !' occas ion d'un bilan sang uin ( I). En 1988, nous avons em is 1· hy poth ese que la grossesse per .\'(:' pcut constitucr unc situati on phys iolog ique au cours de laq ucll e la th yro·1·de mat erncll e est so umi sc pend ant plu sicurs mois a un c stimulati on excess i ve. En effet, la grossessc cntralne des alterations pro fondes de I' eco nomi c 1hyro·1·d icnne globale, en raison de I' int erve ntion de trois factcurs indepcndanl s qui concourent 11 modi fi cr l 'eq uilibre th yro'i'di en chez la femmec ncc inte. L e pre mi er facte ur res ullc de ! 'e levati on import anle des taux circulant s de la proteine vec trice principalc rcs ponsabl e du transport sanguin des hormo nes th yro'i'di c nn cs, l a th yrox in e-b indin g globuline (TBG). L e deux iemc fac teur est li e 11 l' c ffct th y reotrope («TSH -likc») de ! ' horm one chori o-go nadotropc (hCG), prod uite en qu anti les rnass i vcs au co urs de la grossesse, princ ipal cment pend ant le premie r trimes trc. Cc tlc hormone est susceptibl e de stimul er directemcnt la glande th y ro·1·dc, modifi ant des !ors la bouc lc de retro-contra le physio log iquc, I' axe hypophysoth yro'idi cn.
