A Mahommedan, named Sharaf Din, aged 30 years, was admitted for calculus into the Sialkote Civil Hospital 011 the 25th July 1891. Previous History.?For his previous history I am indebted to Assistant-Surgeon Mehta Dunni Chand. He came to the Sialkote Hospital on the 30th January 1891, complainingof retention of urine, A silver cathewhich had lasted for one night. ter was passed, and about a pint of urine was As he still removed. No stone was detected. had difficulty in passing urine naturally, a soft catheter was again passed without difficulty. His urine contained mucus and phosphates. He was orderd tonics, strychnia, and good diet, while the bladder was regularly washed out with boracie lotion. His condition remained unaffected by this treatment. He was supplied with a vulcanised india-rubber catheter and was taught its use. About eight days after his admission his urine passed naturally. On the 17th there was a sudden stoppage to the flow of his urine, and he at once passed the catheter as before, removing all his urine. However, on withdrawing the instrument only about half of it came away, the remainder presumably being left in his bladder. He was at once put under chloroform, and a search was made for the missing portion. A urethral calculus was immediately detected and removed by an external incision. This calculus was about half inch long by one-fourth inch broad and was cylindrical in shape. Its nature is not mentioned. No trace of the missing portion of the catheter could be discovered. The man was discharged on the 27th February 1891, and continued free from any vesical symptoms for about three months when he again experienced difficulty iu passing his urine. He was re-admitted on the 25th July 1891, for vesical calculus. On the 27th July 1891 assisted by Assistant-Surgeon Dunni Chand, I attempted to remove the calculus by litholapaxy, but had soon to desist, as the calculus seemed fixed in a pouch to the left of the neck of the bladder. Lithotomy was then performed in the usual manner, and a phosphatic calculus with the retained part of the catheter as its nucleus was removed.
