Carcinofetal Antigens. I. Alpha-Fetoprotein Summary. Alpha-fetoprotein (AFP) is an atphal-glycoprotein (M.W. about 65 000) appearing in the fetal serum of most mammals including man during the early stages of pregnancy; 4 weeks after birth it disappears altogether or exists at very low concentrations as in the normal adult. AFP is formed in the yolk sac, the fetal liver and the gastro-intestinal tract. One of its physiological functions in fetal life is supposed to be the protection of the fetus from maternal oestrogens (oestrophilic property). The clinical significance of AFP is based on the regular and increasing production in primary liver cell carcinoma, less frequently in teratogenetic tumors where it serves as a control of therapy and course of the disease. Less frequent, minor and temporary increases in the AFP serum level occur in several primary tumors with secondary liver involvement, and in inflammatory gastro-intestinal diseases, e.g. of the liver (hepatitis, cirrhosis). AFP has an increasing importance in gynecology (gestational age, fetal distress syndrom, malformations, hydatidiform mole/chorion carcinoma). The physico-chemical properties of AFP are widely known. Both fetal and tumor AFP appear to be immunologically and biochemically identical, as are that of tissue and biological fluids. The differences observed (variants, microheterogeneity) depend mainly on the different content of sialic acid. An antigenetic relationship exists, between the AFP of most species. The immunodiffusion (Ouchterlony) is the most frequently used but relatively insensitive test ( 1 - 5 gg/ml) in finding AFP, whereas the radioimmunoassay is the most sensitive one (up to 0,25 ng/ml) and permits the determination of normal serum levels in adults (below 20 ng/ml). The serum concentration in healthy pregnant women lies up to 500 ng/ml, in patients with hepatitis, liver cirrhosis and other liver diseases mostly under 3gg/ml, whereas in those with primary liver cell carcinoma levels up to and above 600 rag-% have been found. Einleitung Eine Beziehung zwischen Immunologie und Krebs beruht auf der Vorstellung, dab Krebszellen durch den Immun-l~berwachungsmechanismus der cellulfiren Immunabwehr als wirtsfremd erkannt werden k6nnen, dal3 das Auftreten yon Krebs mit der Immunkompetenz des Wirtes korreliert und ein immuntherapeuti o scher Angriff auf den Tumor zum Nutzen des Patienten m6glich ist [89, 176, 279, 314, 348,466]. Demnach soll sich die Oberfl/iche von Krebszellen gegeniiber

wie auch beim Menschen schon frfih w/ihrend der Gestation im Feten auftritt und etwa 4 Wochen nach Geburt im Neugeborenen wie sp~iter beim gesunden Erwachsenen nicht mehr oder nur in sehr geringer Konzentration nachweisbar ist. Es wird im Dottersack, der fetalen Leber und dem fetalen Gastrointestinaltrakt gebildet. Seine physiologische Bedeutung im Fetalleben besteht m6glicherweise u.a. im Schutz des Feten vor den mfitterlichen ()strogenen (6strophile Eigenschaft), Die klinische Bedeutung von AFP beruht auf seiner regelmfiBigen vermehrten und zunehmenden Bildung beim prim~ren Leberzellcarcinom, seltener bei teratogenetischen Geschwfilsten. Dort dient es vornehmlich zur Therapie- und Verlaufskontrolle. SeRene, geringere und mehr vorfibergehende Erh6hungen des AFP-Serumspiegels fanden sich bei einigen anderen Prim/irtumoren mit sekund~irer Leberbeteiligung und bei entztindlichen gastrointestinalen Erkrankungen vor allem der Leber (ttepatitis, Cirrhose). Neuere Untersuchungen weisen auch auf eine wachsende Bedeutung yon AFP ftir die Gyn~ikotogie hin (Gestationsalter, fetal distress-Syndrom, Mil3bitdungen, Blasenmole/Chorioncarcinom). Die physikalisch-chemischen Eigenschaften yon AFP sind weitgehend bekannt. Fetal- wie Tumor-AFP scheinen immunologisch und biochemisch identisch, ebenso Gewebs-AFP mit dem aus biologischen Fltissigkeiten. Beobachtete Unterschiede (Varianten, Mikroheterogenit/it) sind hauptsgchlich durch unterschiedlichen Sialins/iuregehalt bedingt. Zwischen Alpha-Fetoproteinen verschiedener Spezies besteht gr613tenteils antigenetische Verwandtschaft. Von den Nachweisverfahren fiir AFP in biologischen Flfissigkeiten ist die Immunodiffusion der verbreiteste aber auch unempfindlichste Test (1 - 5 ~tg/ml), w~ihrend der Radioimmunoassay die gr6Bte EmpfindIichkeit (bis 0,25 ng/ml) aufweist und auch Erwachsenen-Serumnormalspiegel (unter 20 ng/ml) zu bestimmen erlaubt. Semmkonzentrationen liegen bei gesunden Schwangeren bis 550 ng/ml (III. Trimenon), bei Hepatitiden, Lebercirrhosen und anderen Lebererkrankungen meist unter 3 gg/ml, w~ihrend beim primfiren Leberzellcarcinora Erh6hungen bis fiber 600 mg-% gefunden wurden. normalen Zellen durch ein abweichendes Antigenprofil unterscheiden, das durch Fehlen normaler, Wiederauftreten ontogenetisch alter oder ortsfremder bzw. Auftreten neuer Antigendeterminanten charakterisiert ist. Das Vorkommen sogenannter tumorspezifischer Transplantationsantigene (TSTA) ist fiir zahlreiche experimentelle Tiertumoren durch Untersuchungen in syngenen Tierstfimmen hinreichend belegt (~)bersicht bei [212]). Im aUgemeinen zeigen chemisch induzierte Tumoren [143, 205, 206, 309, 314] individuelle nicht kreuzreagierende TSTA, w/ihrend virusinduzierte Tu-

moren [101,147] des gleichen Virus miteinander antigenetische Verwandtschaft ihrer TSTA aufweisen [127, 284, 314]. Beim Menschen konnte neben Hinweisen auf die Virusgenese einiger Tumoren [19, 42, 61, 98, 166, 182, 269, 278] durch andere experimentelle Methoden gezeigt werden, dab viele unterschiedliche Tumortypen wie z.B. Colon-, Ovarial-, Bronchial-, Mamma- und urotheliale Tumoren, Neuroblastome, Melanome, Lymphome, Leumfimie und Sarkome tumorassoziierte Antigene enthalten [81, 89, 93, 95, 310, 348]. Zu diesen geh6ren auch die carcinoembryonalen, embryospezifischen, carcinofetalen oder onkofetalen Antigene, d.h. Antigene, die im Verlauf der intrauterihen Entwicklung yon Mensch und Tier in der Placenta oder in fetalen Organen gebildet werden und nach Geburt nicht mehr oder in sehr geringer Konzentration nachweisbar sind [17, 40, 74, 79, 80, 144, 226, 242]. Im weiteren Sinne zfihten auch fetale Enzyme, die erst unter Tumorentwicklung wieder im gleichen Gewebe auftreten wie z.B. beim Hepatom Aldolasen, LDH, Hexokinasen und Pyruvatkinase zu den carcinofetalen Substanzen [367], ferner in Tumoren ektopisch gebildete Hormone [40]. Die Bildung solcher Antigene wurde schon 1929 yon Hirszfeld [164, 165] vermutet und geschieht wahrscheinlich als retrogenetische Expression [424] bzw. durch Deletion oder Derepression eines reprimierten Genoms [146]. Carcinofetale Antigene sind in zahlreichen Tiertumoren beschrieben worden [212, 344] und gewinnen auch beim Menschen zunehmend an Bedeutung, da man sich durch ihre Kontrolle eine Frfihdiagnose bestimmter Tumoren, zumindest jedoch eine erfolgreichere Beurteilung therapeutischer Mal3nahmen erhofft [264]. Nach Alexander [17] lassen sich die onkofetalen Antigene (OFA) nach ihrer Verbreitung in assoziierte (zusfitzlich nachweisbar in einigen normalen Erwachsenen-Geweben), heterospezifische (zus~itzlich prfisent im spMen Fetalleben und deshalb immunologisch toleriert) und solche einteiten, die nur im frfihen Fetalleben oder sp~ter getarnt vorkommen und im Tumor intracytoplasmatisch (OFA-autospezifisch) oder auf der Zellmembran (Transplantations-Typ) liegen. Nach dieser Einteilung geh6ren die beim Menschen vorkommenden onkofetalen Antigene wie z.B. das carcinoembryonale Antigen (CEA) und Alpha-Fetoprotein (AFP) vorwiegend zum assoziierten Antigentyp. Im folgenden werden die bekanntesten humanen carcinofetaten Antigene beschrieben. Alpha-Fctoprotein (AFP)

Nach der Entdeckung yon Fetuin in fetalem K/ilberserum [Pedersen 1944:315, 60] und yon AFP dutch Bergstrand und Czar [5t, 52] sowie anderen [77, 126,

152, 251, 282, 434] in fetalem menschlichen Serum beobachteten Abelev u.Mitarb. 1963 [1] sowie sp~iter andere [148, 194] ein fetales Alphal-Globulin im Serum yon erwachsenen M/iusen und Ratten nach Hepatektomie und bei Tr~igern von transplantierbaren Hepatomen [373, 421, 422, 463], ferner bei Rindern mit Leberzell-Carcinom [203]. Ein analoges AFP wurde zun~ichst von Tatarinov [431 -433] bei Patienten mit primfirem LeberzeU-Carcinom und yon Abelev et al. [7] bei malignen Teratoblastomen des Hodens nnd Ovars sowie auch nach partieller Hepatektomie [224, 263] nachgewiesen (l~lbersicht bei [4, 7, 78, 125, 130, 160, 260, 401,445]). AFP wurde inzwischen im Serum yon 18 Sfiugetierspecies gefunden, z.B. bei Maus, Ratte, Meerschweinchen, Goldhamster, Kaninchen, Katze, Hund, Ziege, Schaf, Schwein, Rind und Pferd [66, 137, 195, 199, 200, 202, 203, 253, 296, 311, 319, 320, 373, 377, 405, 42t, 422, 448,473]. Human-AFP wird ab der 4. Schwangerschaftswoche im Feten nachweisbar [388] und erreicht im Fetalserum die hSchste Konzentration um die 13. - 15. Entwicklungswoche ( 3 - 4 mg/ml, [135, 375]). Der Serumspiegel f/illt gegen Ende der Schwangerschaft stark ab, um etwa 2 - 5 Wochen nach Geburt fast ganz zu verschwinden [75, 376]. Mit empfindlichen radioimmunologischen Methoden lfiBt sich auch bei normalen Erwachsenen noch ein Spiegel zwischen 2 und 25 ng/ml [352, 381] finden, ferner in h6herer Konzentration bei Schwangeren [7, 92, 120, 382]. Bei anderen S~iugetieren (Meerschweinchen, Hund, Katze, Kaninchen, Goldhamster) verschwand AFP unter Immunodiffusionskontrolle in obiger Reihenfolge nach 3, 20, 10, 16 und 10 Tagen nach Geburt in gewisser Abhfingigkeit yon der relativen Reife des neugeborenen Sfiugetiers [473]. Vorkommen von AFP Als Bildungsort von AFP gilt bei Ratte [136, 139, 287], Kaninchen [402], Huhn und anch beim Menschen die fetale Leber, der Dottersack und fetale Gastrointestinaltrakt mit einer maximalen Konzentration zwischen der 10. und 20. Schwangerschaftswoche [1t0, 135, 138-142, 245, 323]. Sein Nachweis gelang nicht in fetalen menschlichen und Ratten-Zellkulturen yon Hirn, Lunge, Herz, Niere, Darm, Skelettmusket und Placenta [139], jedoch in fetalen menschlichen und Mfiuse- [247] Leberkulturen, in menschlichen Hepatomzellkulturen [347], in transplantierbaren M~iuseteratocarcinomen [112], nach anderen in der Placenta [124], in der perfundierten isolierten humanen fetalen Leber [191] und radioimmunoelektrophoretisch in humanem fetalen (Leber, Thymus, Magen, Colon) und metastasierenden Hepatomgewebe [459]. Immunofluorescenzoptisch fand sich AFP in humanen Leber-

parenchymzellen des Feten und Neugeborenen sowie in Tumorzellen yon Patienten mit primfirem Leberzellcarcinom [85, 111,298, 301,323, 324, 406, 428, 429]. Zwischen dem AFP-Serumspiegel und dem Auftreten und Ausmal3 der Fluorescenz in den Tumorzellen bestand keine Korrelation noch zwischen diesem und der histologischen Gradeinteilung des prim~iren Leberzellcarcinoms nach Edmondson und Steiner [103,301], w/ihrend Takahashi et at. [430] eine intensive Fluorescenz bei Hepatom mit tiberschiel3end basophilem feingranuliertem Cytoplasma bei leichter bis manger Differenzierung (Grad I I - I I I ) in Verbindung mit hohen Serum-AFP-Werten beschrieben. AFP lag in fetalen Hepatocyten und Hepatomzellen diffus intracytoplasmatisch [100, 110, 111,245, 298, 301,324], bei Hepatomzellen zus~ttzlich in der Cytoplasmamembran und der perinucle/iren Zone [298, 301]. AFP-positive Zellen zeigten manchmal acidophiles granulfires Cytoplasma mit gr6Beren hyperchromen Kernen [298, 301 ]. Shikata und Sakakibara [393] wiesen in experimentell induzierten Hepatomzellen der Ratte mit Hilfe der Peroxydase-Antik6rpertechnik AFP elektronenmikroskopisch vorwiegend linear im rauhen endoplasmatischen Reticulum an der Oberfl/iche wie auch um freie Ribosomen, manchmal in Teilen des ~iul3eren Lagers der Kernmembran sowie auch teilweise im weichen endoplasmatischen Reticulum nach, weshalb sie AFP als Sekretionsprodukt betrachten. Demgegenfiber fanden Uriel u.Mitarb, durch Zellaffinit~tsmessung mit tritiummarkierten Ostrogenen AFP autoradiographisch in menschlichem primgtren Leberzellcarcinom [447, 449] wie Rattenhepatom [444] weder in normalen noch malignen Leberzellen, sondern in fJ'bergangszellen (kleinen runden bis ovalen Zellen mit elongiertem oder nierenf6rmigem Nucleolus), die an der normalen Leberregeneration teilnehmen, in I - 2 % der gesamten Leberzellpopulation. Daher vertreten sie die Hypothese, dab die Bildung und Sekretion yon AFP nur im ()bergangsstadium der Leberzelldifferenzierung auRritt. Das yon Dufour u.Mitarb. [100] bei Rattenembryonen im Fruchtwasser gefundene AFP stammte aus der erythroblastischen Linie von embryonaler Leber und Blur. Im Rahmen der Tyrosin/imie mit AFP-Nachweis in Serum [48] und Organextrakten wie z.B. Leber, Lunge, Niere, Herz, Milz und Thymus [49] diskutiert auch Belanger Beziehungen zwischen AFP-positiven Hepatocyten und Blut- oder Lymphgewebe. Nach anderen Untersuchern zeigte sich bei regenerierender Leber bzw. bei Hepatompr/iparaten nur eine begrenzte Zahl AFP-positiver Zellen (bis 5% : [145,410] bzw. 20% : [298,301]), die in dreidimensionalen Gruppen nahe bei Blutgef/iBen und Capillaren [6, 145] bzw. gleichmfil3ig verteilt [298, 301] lagen. Bei fetaler Leber war AFP hauptsfichlich in den periportalen Parenchymzellen um die Zentralvenen gelagert [7, 110, 406]. Beztiglich der AFP-Gesamtkonzentration in Hepatomen fanden Lehmann u.Mitarb. [234, 241] bei 8 untersuchten Ffillen zwischen 12 gg und 630 pg/g Feuchtgewicht. Schlieglich wurde AFP in Serum, Ascites, Stuhl, Urin [135, 198, 235, 245, 376, 452], in Galle und Cerebrospinalfltissigkeit [452], in der Pteurafl/issigkeit yon Hepatompatienten mit Lungenmetastasen [235, 265,407] und bei Feten im Fruchtwasser (Mensch: [91, t35, 208,384, 388, 452]; Ratte: [100, 287]) gefunden.

Physiologische Bedeutung yon AFP und Atiologie des Wiederauftretens Im Fetalleben scheint AFP zunfichst die erst anlaufende Albuminbildung weitgehend zu ersetzen. Die

nachgewiesene Ostrogenbindung durch AFP ([306, 446-449], Ratte: [37, 444]) wurde ats m6gliche Schutzfunktion f/ir den Feten gegen/iber den mtitterlichen lJstrogenen gedeutet [80]. Aus diesem Grunde wurde diskutiert [167], ob das beim Erwachsenen beobachtete hfiufigere Auftreten yon benignen Hepatomen unter vermehrter Ostrogenstimulation der Leber (z.B. bei Einnahme yon Contraceptiva) nicht durch den fehlenden AFP-Schutz begtinstigt wird. Andere Untersucher fanden tierexperimentell durch AFP-Neutralisation mittels Verabreichung yon Anti-AFP-Serum an schwangere Tiere [403, 741] oder in befruchtete Eier [741] eine hohe fetale Mortalitfitsrate, Dottersackschfidigung und Mil3bildungen, weshalb sie AFP eine wichtige Rolle in der Organogenese zuschreiben. Ftir das spfiter vermehrt gebildete AFP werden verschiedene Hypothesen angenommen [9, 260, 262]: Nach Abelev [3, 4, 9] sowie Lacassagne [222] wird die genetische Information zur AFP-Bildung durch Persistenz spezieller Hepatoblasten beim Hepatom bzw. Dottersack-Entodermfibnlicher Zellen beim Teratom [9, 57, 207, 437], nach Uriel [442, 443] durch Rtickdifferenzierung von Hepatocyten oder nach Pierce [3t 8] durch Verbleiben von noch undifferenzierten Stammzellen gedeutet. Nach Grabar [146] lfil3t sich auch eine Neosynthese eines fetalen Antigens dadurch erkl~iren, dab durch Synthesehemmung eines als Repressor agierenden Proteins die Derepression eines zuvor reprimierten Genoms stattfindet. Ftir das Auftreten von AFP bei entztindlichen Lebererkrankungen wird meist eine Leberregeneration verantwortlich gemacht [7, 448]. Ishii [180] nimmt an, dab es sich dabei um eine reversible genetische Ver/inderung gegenfiber einer irreversiblen bei der Zellcancerisation des Hepatoms handelt.

Experimentell induzierte AFP-Bildung lm Tierversuch 1/it3t sich AFP-Bitdung durch akute Vergiftung mit Tetrachlorkohlenstoff [41, 66, 316, 345] sowie durch Verabreichung einer Vielzahl yon chemischen z.T. cancerogenen Substanzen induzieren, wobei es entweder zu einer vorfibergehenden Schfidigung oder nach 1/ingerer Einwirkung zu Hepatombildung kommt, ferner auch bei spontanen oder transplantierbaren Hepatomen (Maus: [148,194]; Ratte: [I6, 43, 96, I59, 177, 196, 197, 216 -219, 287-289, 311, 313, 372, 393, 408, 422, 462-464]; ohne AFP: [421, 422]; Kaninchen: [66]; Affen: [118, 120, 121, 17t, 172, 335]; Hund: [395]; Rind : [204]). Dabei kann man gew6hnlich eine ,,prim/ire Reaktion '° (Leberzeltsch/idigung, [159]) mit einem passageren AFP-Anstieg im Blut (etwa zwischen 3. und 11. Woche: [159, 197, 216, 313]) von einem dauerhaften Wiederanstieg im Zusammenhang mit Hepatombildung (z.B. nach der 12. Woche oder sp~iter: [159, 197]) unterscheiden. Die AFP-Bildung findet wahrscheinlich in der ersten Phase in den ovalen Zellen [197] bzw. Ubergangszellen und den kleinen neuen Hepatocyten [313] sowie sp~tter in den Hepatomzellen [197] statt. SchlieBlich konnten auch humane Lebergewebskulturen aus

Leberbiopsiepr~iparaten durch Inkubation mit Acetylaminofluoren und Tannins~iure nach 70 Tagen zur AFP-Bildung angeregt werden [4081.

Physikalisch-chemische Eigenschaften yon AFP

Humanes fetates wie Hepatom-AFP ist ein Glykoproteid yon elektrophoretischer Alpha~-Beweglichkeit, einem Molekulargewicht zwischen 61 000 und 75000 [11, 25, 28, 159, 255, 294, 350, 351, 353] und einem Sedimentationskoeffizienten zwischen 4,5 und 5,5 S [135, 255,293, 294]. Ratten-AFP besitzt ein Mo]ekulargewicht yon 70 000 bzw. 72 000, einen Sedimentationskoeffizienten von 4,5 S u n d einen isoelektrischen Punkt yon 4,9 [37, 368]. Der molare Extinktionskoeffizient yon Human-AFP bei 278 mg betrfigt EI'~* - ~ a (Fetus) bzw. 5,26 (Hepatom, [159]), sein isoelektrischer Punkt liegt zwischen 4,7 und 5 [25, 255, 350]. Sein partielles spezifisches Volumen betrfigt Vzo= 0,726 ml/g und sein Diffusionskoeffizient 6,18 x 10- 7cm2/sec. Die elektrophoretische Mobilitfit wird mit -6,08 x 10- 5cm2V tsec- 1 angegeben [255]. Nach Untersuchungen yon Masopust u.Mitarb. [255] ist AFP in 0,4%iger Rivanoll6sung unl6slich, aber 16slich in 0,1 g-%. Vollst/indiges Aussalzen findet in 2,8 M Ammoniumsulfat-L6sung statt. Auch in 0,15 M Trichloressigsfiure, in 0,6 M Perchlorsgure sowie in 20%iger Sulphosalicyls~iure ist AFP nicht 16slich [255]. Der Kohlehydratgehalt (vor allem Mannose, Galactose, Gtucosamin und Sialinsfiure, [159]) wurde zwischen 3 und 4,3% [159, 294, 350, 351, 353] mit einem Hexose-Hexosamin-Sialinsfiure-Verh/iltnis von 2,2:1,2:0,9 bestimmt [351]. Die Aminos/inren-Analysen verschiedener Autoren [159, 294, 350, 351] yon Hepatom- wie fetalem AFP zeigen nahezu gbereinstimmende Ergebnisse. Tumor-AFP soll bis zu 4 Varianten and Subkomponenten enthalten, deren Unterschiede dutch Behandlung mit Neuraminidase and Entfernung der Sialinsfiure weitgehend aufgehoben werden [331,334, 338]. Alpert u. Mitarb. [28, 29] fanden durch Isoelektrofokussierung, Ionenaustauscherchromatographie mad Agaroseget-Elektrophorese 2 Hauptformen von Hepatom- wie Fetal-AFP, die sich durch ihre isoelektrischen Pmakte (4,82 und 5,2) unterschieden. Neuraminidase-Behandlung bewirkte eine Umwandlung zu einer homogenen Form mit langsamerer elektrophoretischer Mobilifftt. Ahnliche Befmade wurden auch bei einem Fall yon Magencarcinom mit Lebermetastasen und AFP-Bildmag erhoben [277] sowie yon Aussel et al. [37] ffir Ratten-Fetal-AFP. Durch Affinit/itschromatographie mit Concanavalin A-Agarose gelang Smith und Kelleher [405] die Trennung yon 2 mikromolekularen AFP-Varianten (Rattenfetalserum, humanes Hepatom), wobei die Concanavalin A bindende Variante eine ~nderungihrer elektrophoretischen Mobilit~it bei kompletter immunologischer Identit~it erfuhr. Ebenso fanden Zimmerman mad Madappally [472] die mikroheterogenen AFP-Fraktionen im fetalen Plasma und Fruchtwasser der Maus haupts/ichlich durch Menge und Bindungstyp yon Sialins~ure an AFP bedingt, wobei die entwicklungsabhgngige St/irke der Sialtransferasen-Aktivit/it in Leber and Dottersack die entscheidende Rolle spielte. Auf eine )~hnlichkeit der physikalisch-chemischen Eigenschaften yon Albumin und AFP wurde yon Abelev [7] hingewiesen. So kann AFP auch Ostrogene [37, 306, 446, 448] binden, nicht jedoch Testosteron [37, 446] oder Progesteron [37]. AFP unterscheidet sich yore Kfilber- und Schaf-Fetuin durch den Kohlenhydratgehalt (Feruin 20% bzw. 22,8%), die Aminos~urenzusammensetzung (Fetuin

ohne Methionin), das Molekulargewicht (Fetuin 48 400 bzw. 48500), den Diffusionskoeffizienten (Fetuin ca. 5,7 x 10-7: [50]), Sedimentationskoeffizienten (Fetuin 3,5 S), den Hexosegehatt (Fetuin 8,3 bzw. 7,5%: [250, 418,419]) nnd eine fehlende Antitrypsinaktivit~it [3531.

Die Halbwertzeit von AFP wurde bei einem Patienten nach totaler Hepatektomie und orthotoper Lebertransplantation zu 2 Tagen [26], nach Teilhepatektomie bei Hepatom zu 5, 7 Tagen [221] bzw. 5 - 8 Tagen [159,224], mit J 1a I_AFP zu 6,2 Tagen [159], haupts/ichlich aber bei post partum-Frauen zwischen 2,5 und 9 Tagen bestimmt [56, 135, 179, 225, 265, 268, 305, 357, 382, 388]. Die Fetoproteinsyntheserate lag in der isolierten perfundierten fetalen Humanleber mit C 14markiertem Leucin zwischen 19 und 26 gg/rnin [191]. Dem entspricht auch ein schneller Einbau radioaktiv markierter Aminosguren in AFP aus fetalen Leberzellkulturen [124, 135]. Die Identitfit zwischen fetalem und Hepatom- bzw. Teratom-AFP konnte biochemisch [150, 159, 294, 295, 350, 351] sowie immunologisch [2, 7, 159, 294, 351, 353, 431 ] nachgewiesen werden. Ferner zeigten biochemische und irnmunologische Befunde keine Unterschiede zwischen Hepatomgewebs- and ausgeschwemmtem AFP in biologischen Fliissigkeiten [235]. Mit potenten Antiseren liel3en sich deutliche Kreuzreaktionen zwischen AFP verschiedener Species wie Mensch, Rind, Schwein, Pferd, Hund, Affe, Katze, Schaf, Kaninchen, Maus und Ratte feststellen [7, 159, 296, 319, 452]. l~ber genetische Varianten von fetalem und Tumor-AFP ist bisher wenig bekannt. Erw/ihnenswert sind Befmade yon Purves et al. [328] und Portugal u.Mitarb. [322], die eine etektrophoretisch langsame Variante von humanem AFP beschrieben. Demgegenfiber beobachteten Ganrot et al. [128] in weniger als 2% der F~itle yon Hepatompatienten eine besonders schnelle AFP-Fraktion. Die Befunde yon Smith und Kelleher [405] fiber 2 molekulare AFP-Varianten sprechen ebenfalls ffir m6gliche fmaktionelle und evtl. genetische Unterschiede.

Reinigung yon AFP Eine Reindarstellung von AFP erfolgte 1970 durch Hirai und Nishi [159, 293, 294] sowie durch Lehmann u.Mitarb. [230-232]. l~Tber Teilisoliermagen wurde yon anderen Autoren berichtet [11, 302, 331,342, 350]. Bei den beschriebenen Reinigungsverfahren erstrebt man zun~ichst eine Anreicherung yon AFP z.B. dutch Hitzef~illung aus Gewebe [231] bzw. aus K6rperflfissigkeiten wie Plasma, Serum, Ascites durch fraktionierte AmmoniumsulfatfNlung [230232, 302], Ammoniumsulfatfallung in 34%iger [297, 298] oder 60%iger L6sung [342] (AFP jeweils im Oberstand), durch F~illung in 0,4%iger Rivanoll6sung [275], AFP im Pr/icipitat), kombinierte Rivanol-Ammoniumsulfat-Fraktionierung [180], Euglobulinpr~icipitation [342] und Calciumphosphatabsorption [230-232]. Weitere Reinigung bzw. Anreicherung geschieht durch Anionenaustauscherchromatographie (DEAE-Zellulose : [342]; QAE-Sephadex A 50: [302, 331]; DEAE-Sephadex A 50: [230-232, 331]), Gelfiltra-

Ats abschlieBender Schritt wird meist eine Immunpr/icipitation [149, 350] manchmal gegen Verunreinigungen [t 80, 230-232, 302] und weitere Trennung durch Ammoniumsulfat [304], oft aber mit monospezifischem Antiserum bzw. Immunglobulin gegen AFP gew~ihlt,z.T. unter Verwendung yon durch Polymerisation unl6slich gemachtem Antik6rper (z.B. nach Avrameas und Ternynck [38, 331]) oder nach der Immunosorbenttechnik mit CNBr-aktivierter Sepharose 6 B [159, 295] oder modifizierter Agarose (Phloroglucinohydroxypropyl-Agarose und CNBr) [84]. Eine Wiederaufl6sung der Pr~icipitate ist in 0,1 M Glycin-HC1-Puffer pH 1,8 It 59, 293,294], in 0,33 M Na-Citratpuffer pH 2,5 [350] bzw. 0,05 M Tris-HC1-Puffer mit 4 M Na-Jodid pH 9 mit nachfolgender schrittweiser Ausffdllung von AFP durch Ammoniumsulfat bis 40% [11] m6glich,

Screening- und Verlaufsuntersuchungen. Bisher liegen nur wenige Ver6ffentlichungen fiber AFP-RIA-Untersuchungen vor. Die verwendeten Testmethoden arbeiten meist nach der Doppelantik6rpertechnik mit AntiAFP-Serum vom Schaf [88,352] oder Kaninchen [179, 180, 292] und Anti-Schaf- oder -Kaninchen-IgG vom Kaninchen bzw. von der Ziege. Dem gleichen Prinzip folgt auch ein handelsfiblicher Radioimmunoassay 3, der nach unserer Erfahrung eine gute Reproduzierbarkeit bei einer unteren Empfindlichkeit yon 5 ng/ml aufweist [224].

Klinische Bedeutung von A F P

Normalwerte Bestimmung von AFP

Zu den wichtigsten Nachweismethoden in K6rperfltissigkeiten, vor allem im Serum, geh6ren mit steigender Empfindlichkeit die zweidimensionale Doppeldiffusion (Ouchterlony) mit nativem Serum (Empfindlichkeit 1 - 5 gg/ml, [233,238]) bzw. nach 4-5facher Anreicherung durch Rivanolfgllung [275] oder durch Verwendung yon MINICON A 25-FiltrationsgeffiBen1; Fluoroelektroimmunodiffusion (0,5 gg/ml, [298]); Elektroimmunodiffusion ( - 350 ng/ml, [27, 303, 399] bzw. 1 mg/ml: [163] bzw. durch zusfitzliche Autoradiographie bis 20 ng/ml); Uberwanderungselektrophorese (3,6-10gg/ml: [27, 62, 210, 211, 238, 415]); Laurell-Immunelektrophorese (2gg/ml, [302]); Latex-Agglutination (1,6 gg/mt: [30, 238]); Komplementfixation [184]; einfache radiale Immunodiffusion (1 bzw. 50 gg/ml) 2 (mit Autoradiographie 54 ng/ml: [392]); indirekte Immunoradioautographie bzw. Autoradiographie ( 1 5 - 3 0 n g / m l : [6, 7, 104- 106,399]); passive Hfimagglutination (30 ng/ml : [237, 238, 240]) bzw. Aggregat-H/imagglutination (30 ng/ml: [6, 8, 235, 312]); ein enzymbindender Immunoassay (50 ng/ml: [32] bzw. I ng/ml: [47]) sowie der Radioimmunoassay (RIA, 2,5-0,25 ng/ml: [88, 109, I57, 158, 179, 180, 183, 261,292, 293, 329, 336, 337, 352, 355, 357, 382]; Ratte: [307, 369, 370]). Bei Untersuchung von Patientenseren in der zweidimensionalen Doppeldiffusion muB auf die unbedingte Mitffihrung einer positiven Kontrolle hingewiesen werden [400]. Wegen der niedrigen Konzentration yon AFP in Normalffillen und bei einzelnen Erkrankungen sowie der wfinschenswerten genauen Konzentrationsangabe eignet sich der Radioimmunoassay besonders gut zu t Fa. Amicon N.Y. Holland. 2 AFP-LC-Partigen bzw. -M-Partigen-Platten d. Fa. BehringWerke, Marburg.

AFP-Konzentrationsangaben ffir Normalpersonen liegen nur ffir radioimmunologische Bestimmungen vor. Ruoslahti u.Mitarb. [352] fanden bei 32 Normalpersonen eine angenfiherte Normatverteilung mit einem Normalbereich yon 1 - 1 4 ng/ml (7,6_+6,6). Purves u.Mitarb. [339] beschrieben eine Log-Normalverteilung mit Werten bis 16,5 ng/ml. Nach Ishiguro et al. [179] wurden bei 36 gesunden nicht schwangeren japanischen Frauen Spiegel unter 7 ng/ml, nach Chayvialle und Ganguli [88] bei 105 klinisch gesunden Personen Spiegel um 2,3_+ 1,2 ng/ml, nach Lafon et al. [223] bei 100 normalen Btutspendern Werte unter 5 ng/ ml gemessen. (AFP-Spiegel bei Schwangeren und Neugeborenen s. weiter unten.) Maligne Erkrankungen

Beim prim~tren Leberzellcarcinom, das in Europa [239, 390], den USA [34, 44] wie in Japan [274] vornehmlich auf dem Boden einer Cirrhose entsteht, wurde AFP mit der Mikro-Ouchterlony-Technik bei 4 0 - 7 0 % , mit der Elektroimmunodiffusion bis 80% der F/ille gefunden (Tabelle 1 ; [33, 87, 122, 128, 130, 154, 163, 168, 198, 209, 252, 271,280, 290, 321, 325, 467, 470]). Mit steigender Empfindlichkeit der Methoden nimmt auch der Prozentsatz positiver Ergebnisse bei Hepatompatienten zu, z.B. nach Lehmann et al. [238] von 58% bei der Doppeldiffusion fiber 68% (Latex-Agglutination), 81% (l~berwanderungselektrophorese) bis nahe 100% bei der passiven Hfimagglutination und dem RIA. Auf die verschiedenen Bestimmungsmethoden mit ihrer unterschiedlichen Empfindlichkeit, verschiedenen Standards und Antiseren dfirfte ein Teil der recht streuenden Angaben fiber AFP-Posivitfit zurfickzuftihren sein [31]. Bisher liegen nur wenige Befunde tiber radioimmunologische AFP-Bestimmungen bei Patienten mit pfimfirem Leberzetlcarcinom vor (Tabelle 2, [59, 88, 180, 292, 354, 364]), Nach Ruoslahti u.Mitarb. [354] konnte bei allen 14 untersuchten Hepatompatienten AFP nachgewiesen werden, davon bei 10 Patienten mit Werten zwischen 3,5 und 840 gg/ml. Nach anderen [88] wurden yon 22 Hepatompatienten bei 17 Spiegel fiber 1000 ng/ml im Plasma gefunden. 90% der von Nishi

et al. [158,292] und Ito et al. [181] untersuchten Ffille zeigten Konzentrationen zwischen 2ng/ml und !0 mg/ml, wfihrend bei Ishii u.Mitarb. [180] 93% der t07 Hepatomtr/iger Werte fiber 15,6 ng/ml und bei Sasaki et at. [364] 27 von 36 Patienten Werte fiber 500 ng/ml aufwiesen. Ebenso lagen bei den Hepatompatienten von Bloomer u.Mitarb. [59] 73% der Spiegel fiber 40 ng/ml. Von unseren 10 beobachteten Hepatompatienten zeigten 9 AFP-Serumspiegel fiber 500 ng/ml und 4 fiber 10 000 ng/ml. Uber/ihnliche Befunde berichteten auch Masseyeff u.Mitarb. [261] sowie Abelev u.Mitarb. [6, 8] ffir die Aggregat-Hfimagglutination. Mit der passiven H/imagglutination fanden Lehmann u.Mitarb. [235, 240] bei allen Hepatompatienten Titer tiber 1:32; Titer fiber 1:512 wurden fast ausnahmslos bei F/illen von Hepatom gefunden. In Prim/irtumoren (Hepatom) und ihren Metastasen lag ein gleich hoher AFP-Gehalt vor. Die AFP-Gewebsspiegel lagen im Vergleich zur Konzentration in den biologischen Fliissigkeiten (Serum, Ascites, Pleuraflfissigkeit etc.) niedrig (zwischen 12 und 630 gg/g Feuchtgewicht, [236]). Auch bei der radioimmunologischen AFP-Bestimmung mit seiner hohen Empfindlichkeit finden sich noch prim/ire Leberzellcarcinome ohne begleitende AFP-Erh6hung im Serum (Werte unter 20 ng/ml), so dab eine Einteilung in AFP-bildende und -nicht-bildende Hepatome auch heute noch gerechtfertigt erscheint [109, 159, 262]. Jfingere Hepatompatienten scheinen h/iufiger einen erh6hten AFP-Spiegel aufzuweisen [27, 39, 75, 239, 257, 262, 267, 286, 445]. Klinische Krankheitszeichen oder biochemische Daten zeigten keine signifikante Beziehung zum AFP-Serumspiegel [27,257, 329, 333]. Eine gewisse Beziehung scheint jedoch zwischen dem AFP-Serumspiegel und dem Cholesterin-, Hb- und Serumproteinspiegel zu bestehen: sie steigen mit dem AFP-Spiegel an, wfihrend Mucoproteine, Alphaz-Globulin [329, 333], AlphalGlobulin und CRP [338] abnehmen. Einige Autoren [26, 27, 330, 333, 359, 364, 393,425, 426, 429] fanden eine Korrelation zwischen dem AFP-Tumorgehalt und der Zunahme der Entdifferenzierung im histologischen Bild, z.B. nach der Klassifikation von Edmondson und Steiner [103, 333, 359, 364, 429, 430], wobei meist Grad II und III bzw. nach Endo [107] oder Miko [273] der anaplastische Typ die h6chsten AFP-Spiegel aufwies. Andere konnten diesen Zusammenhang nicht bestfitigen [257, 301,308, 443]. Nach Shikada und Sakakibara [393] bilden gut differenzierte und hoch anaplastische Hepatome, ferner solche mit geringer und hochgradiger Zellatypie kein oder nur sehr wenig AFP, wfihrend nach Sekurai und Miyaji [359] AFP-Bildung zur geringeren Differenzierung der Gallebildung in Beziehung steht. Ferner wurde eine Korrelation des AFP-

Tumorgehaltes zur AFP-Serumkonzentration [233236, 241] beschrieben, nicht jedoch zwischen dem AFP-Serumspiegel bzw. der Serumkonzentrationszunahme und Tumorgr6ge, Tumorwachstum [7, 109, 262, 301,327, 338, 359, 430, 443, 445], -Stadium, der Schwere der Erkrankung [300, 326, 333,443,445] oder dem MalignitMsgrad [7, 354] entsprechend der Klassifikation nach Broders [73]. Dagegen wurde von Masseyeff u.Mitarb. [257, 262] sowie auch anderen [198, 241,340, 425,426] auf eine zunehmende AFP-H/iufigkeit bzw. steigende AFP-Serumkonzentration [26] mit zunehmendem Tumorgewicht hingewiesen : 50% positive Ffille bei weniger als 2 kg, 100% bei mehr als 5 kg Tumorgewicht. Andere Autoren [257, 258, 333] fanden hier keinen Zusammenhang. Auch kleine Tumoren k6nnen u.U. AFP-Spiegel fiber 1 rag-% hervorrufen [235], wie auch groBe Tumoren geringe AFPKonzentrationen aufweisen [364] oder sogar AFP-negativ sein k6nnen [430]. Uber eineTendenzyon Australia-Antigen-positivenHepatompatienten zu h~iufigererAFP-Positivit/it (Immunodiffusion:[156, 299, 393,417, 455- 457])bzw. h~iufigerAuAg-Positivitfitbei Hepatomtr/igern [82, 151,237, 283, 299, 343, 346, 361, 398, 427, 456, 457] wurde berichtet. Demgegenfiberfanden andere Untersucher [94, 259, 411] keine signifikanth6here AuAg-Frequenzbei Hepatompatientenmit oder ohne AFP-Nachweisbzw. fiberhauptkein AuAg [33, 34, 175, 276, 358, 397, 408, 436, 453, 467], weshalb sie einenursgchlichenZusammenhangzwischenHepatitis und Hepatom nichtffirwahrscheinlichhalten. Braunsteinu.Mitarb. [68]beobachtetenbei 17 von 125 (13,6%) Hepatompatientenaus UgandaektopischeChoriongonadotropinbildung. Die HCG-positiveGruppe war signifikant /ilter und zeigte seltenerAFP-Positivit~it. Ferner wurde eine AFP-Erh6hung im Serum beim Hepatoblastom gefunden[22, 67, 123, 154, 322], bisweilenauch vermiBt [321]. BeiTeratomenundTeratoblastomen(Hoden: [4, 64, 170, 252, 267, 412,438]; Ovar: [252, 450]; retroperitoneal: [13, 170]; mediastinal; sacrococcygeal bzw. pr/isacral: [113, 155,252,266,290,438]) sowie DottersackTumoren (endodermaler Sinustumor, [437]) wurde bei Erwachsenen in 15-20% [170, 268], im Kindesalter bei 7 0 - 8 0 % [75, 265] und insgesamt bei 17-60% [3, 64, 252, 268] der F/ille ein positiver AFP-Befund erhoben [3, 4, 13, 64, 65, 94, 102, 109, 115, 130, 154, 155, 173, 201,246, 248, 252, 256, 266, 300, 412, 414, 423,450, 468,470], nicht jedoch bei Chorionepitheliomen, Seminomen, Dysgerminomen, anderen embryonalen Tumoren, Neuro- und Nephroblastomen, Wilms-Tumoren und embryonalen Sarkomen [3, 7, 268], abgesehen von einem kfirzlich beschriebenen malignen Dysembryom des Ovars [465] sowie ausnahmsweise bei Seminomen [8, 75], Nephroblastom und Chorionepitheliom [8]. Mit der empfindlichen indirekten Immunoradioszintigraphie fanden Elgort u.Mitarb. [106] AFP bei 19 von 21 Patienten mit Hodenteratobla-

Wir fanden radioimmunotogisch bei 41 Patienten mit Hodentumoren in 11 F/illen mit histologisch gesichertem oder wahrscheinlichem Teratom AFP-Serumkonzentrationen fiber 100 ng/ml, wfihrend 29 Ffille mit Werten unter 20 ng/ml fiberwiegend histologisch gesicherte Seminome darstellten. Eine positive Korrelation zwischen AFP-Positivit/it und dem Malignit/itsgrad [268] bzw. eine negative Korrelation zum Alter [267] wurden beobachtet. Finkelstein u.Mitarb. [1 t5] konnten mit Anti-Affen-AFP-Serum bei 7 yon 107 Kindern mit malignen Erkrankungen eine positive Reaktion feststellen, von denen bei Prfifung mit Antihuman-AFP-Serum nur 2 (Teratom, Orchioblastom) eine identische Reaktion zeigten. Esterhay u.Mitarb. [113] beobachteten bei einem 17jfihrigen M/idchen mit histologisch gesichertem Teratocarcinom des Ovars irn Anschlugan eineoperativeund cytostatischeTherapieeine kontinuierliche Abnahme des AFP-Spiegelsauf normale Werte, obwohl es interkurrent zu Tumorrezidivenbetr~chtlichen AusmaBes kam. Wegen der )knderung des histologischen Bildes des Tumors unter Therapie - anfangs vorwiegendunreife neurale Elemente, sp/iter Fehlen dieser Strukturen und fiberwiegend reifes Gliagewebe diskutieren die Autoren, ob die AFP-Bildungbei Teratocarcinomen nicht auf unreife neurale Zellen beschr~inktist. Im wesentlichen liegt die klinische Bedeutung yon AFP-Bestimmungen beim prim/iren Leberzellcarcinora und bei teratogenetischen Tumoren weniger in ihrer Frtiherfassung [83, 389] als vielmehr in der Kontrolle ihres Therapieerfolgs [26, 64, 75, 83, 106, 109, 154, 201,221,238, 243, 244, 257, 265, 268, 333, 338, 340, 426, 438,445,451,457] zur Entdeckung von Residualtumoren, Rezidiven und Metastasierung [65, 426]. Als Reaktionsformen lassen sich unterscheiden [260]: 1. Vollstfindiges Verschwinden yon A F P im Serum als Zeichen der votlst/indigen Tumorentfernung. 2. AFP-Spiegelabfall mit niedriger Plateaubildung und folgendem vollstfindigem Verschwinden als Zeichert einer sprit einsetzenden Zerst6rung AFP-bildender Tumorzellen durch eine Immunantwort. 3. Teilweises oder vollstfindiges Verschwinden von A F P im Serum mit nachfolgendem Wiederauftreten ats Zeichen eines Rezidivs bzw. nach Desarterialisation [162, 169]. 4. Fehlendes oder nur geringes Absinken des AFPSpiegels als Zeichen yon nicht vollstfindiger Tumorentfernung bzw. schon bestehender Metastasierung. Wfihrend eine Beeinflussung des AFP-Serumspiegels bei chirurgischer Behandlung hfiufiger beobachtet ~urde [64, 75, 109, 154, 221,268, 338, 426, 438], sind entsprechende Befunde unter cytostatischer Therapie (z.B. Methotrexat, Cyclophosphamid, Cortison) bzw. Strahlentherapie seltener bzw. fehlen sogar ganz [75, 109, 154, 221,243, 268, 332, 338, 438]. In einzelnen Ffillen wurde AFP meist im Zusammenhang mit sekund/iren Lebermetastasen[8, 54, 62, 234, 235] folgenderPrimer-

tumoren gefunden: Magen- [t2, 24, 54, 55, 58, 63, I06, 109, 133, 134, 161, 180, 213, 257, 258, 272, 277, 292, 301, 339, 354, 365, 366, 430, 470], Oesophagus- [420], Bronchial-, Colon- [35, 94, 180, 223] bzw. Darm- [35, 55, 106], Pankreas- [35, 55, 106, 180, 257, 258,308], Ovarial-[470],Prostata- [272]und Gallenblasen-Carcinom [180, 223, 354], ohne Metastasierung: [56], Hypernephrom ohne Lebermet. [317], ferner bei maligner Lymphogranulomatose[354], maligner Reticulose [356] und anderen Blutkrankheiten [8] sowie manchmal bei Patientinnen mit Blasenmote[179, 379] und Choriocarcinom[379].Demgegenfiberfanden Elgortu.Mitarb. [106]mittels indirekter Immunoradioautographie bei 44 Erwachsenen und 63 Kindern mit malignen Erkrankungen des lymphoreticulfirenSystems kein AFP. Es sollte immer an die M6glichkeit gedacht werden, dab ein Hepatom auch ein sekund/ires Leber-Carcinom vort/iuschen kann [12, 87]. Entsprechend der unterschiedlichen Empfindlichkeit der Untersuchungsmethoden steigt der Prozentsatz yon AFP-positiven Befunden bei anderen Tumoren auger Hepatom und Teratom von 2,1% (Doppeldiffusion)fiber 6,6% (Latex-Agglutination)bis fiber 38% (passiveH~imagglutination)[238]. Nicht maligne Erkrankungen

Vorfibergehende und geringere AFP-Konzentrationserh6hungen im Serum fanden sich bei akuter Virushepatitis [6, 15, 20, 59, 62, 76, 116, 132, 134, 158, 188, 189, 193, 215, 234, 236, 238, 254, 281,292, 300, 301,339, 358, 365, 366, 396, 404, 413, 416] unter Verwendung der Aggregat-Hfimagglutination in 71% [6, 8] und des RIA mit Werten zwischen 6 5 - 4 4 0 [354] bzw. fiber 40 ng/ml bei 27% [59] bzw. fiber 20 ng/ml bei 52% [292] oder 30% und mehr ([180, 296], s. Tabelle 2); ferner bei alkoholischer [59, 233], drogeninduzierter [59], chronisch-aggressiver [180, 234, 235], chronisch-persistierender Hepatitis [6, 8,233, 234], bei primfir-bilifirer [59, 233] und anderer Lebercirrhose (Tabelle 2, [6, 8, 20, 59, 62, 158, 234, 237, 292, 300, 301, 354, 404]); gelegentlich bei Gallengangslfisionen und VerschluBikterus [59], H~mochromatose [t34, 363], Ataxia teleangiectasia [461] sowie beim Down-Syndrom [10]. In einerprospektivenStudiefiberAFP bei Lebercirrhosenunter Verwendung der passiven Hfimagglutination beobachteten Lehmann mMitarb. [237]bei 2t,2% von t42 FfillenpathologischeSpiegel zwischen 0,006 und 0,3 rag-%. 84-95% dieser Ffille zeigten im Verlauf transitorischen AFP-Anstieg and -Abfall, wiihrend bei 7,7-15,4% wachsendeAFP-Konzentrationen eine maligne Leberzelltransformation anzeigten. Eine direkte Beziehung zwischen AuAg und AFP-Erh6hungbei Hepatitidenwurdeyon Purvesu. Mitarb. [339] und Bloomer etal. [59] beschrieben, w/ihrend Ruoslahti mMitarb. [358]sowieSilveret al. [396]sie nicht best/itigen konnten. Nach Karvountzis und Redeker [188, 189] und Silver et al. [396] besteht eine positive Korrelation zwischen dem AFP-Nachweis im Serum und der Schwereder Hepatitis. Bei schwerer Hepatitis wird ein positiverAFP-Nachweisals Ausdruckeinerin~ensivenLeberzellregeneration und sein Fehlen bei einer fulminanten Hepatitis als prognostischschlechtesZeichenangesehen. Bei unkomptizierterHepatitis bestand eine gewisse Korrelation zwischen dem AFP-Spiegel und der Prothrombin-Erniedrigung[189] sowie in zeitlicher Verschiebung zum SGPT-Anstieg[396].

Besondere VerhNtnisse liegen im Kindesalter vor [75, 339]. Bei entziindlichen und toxischen Lebererkrankungen (Virushepatitis, infekti6ser Mononukleose, Galtengangsatresie, Cholostase) ist AFP u,U. bis zum Ende des ersten Lebensjahres auch noch mit der Immunodiffusion nachweisbar [14, 75, 185, 252, 254], ebenfalls bei der hereditfiren Lebercirrhose der Kinder in Indien [86, 285], ferner bei Kindern mit Tyrosinose [45, 46, 48, 49, 75, 76, 99, 268]. Ferner wurde von Zeltzer u.Mitarb. [471] und Kang u.Mitarb. [186] auf signifikant h6here AFP-Spiegel (fiber 4 rag-%) im Serum bei Neugeborenen mit Hepatitis im Gegensatz zu Kindern mit Gallengangsatresie hingewiesen.

AFP und Schwangerschaft Als Ursache ftir die gegentiber der Erwachsenennorm erh6hte AFP-Serumkonzentration bei Schwangeren konnte tierexperimentell bei Ratten [371] und Kaninchen [66] die Bildung yon AFP in der fetalen Leber und seine schnelle Diffusion in den mfitterlichen Kreislauf nachgewiesen werden. Mit gering empfindlichen Methoden (Immunodiffusion, Elektroimmunodiffusion) fanden Alpert u.Mitarb. ([23], USA) wfihrend der 6.-40. Schwangerschaftswoche (SSW) im mfitterlichen Serum kein AFP, wfihrend O'Connor u.Mitarb. ([308], Sfidafrika) es unter gleichen Bedingungen schon nachweisen konnten. Rassische Unterschiede wurden diskutiert [119]. Nach Cohen u. Mitarb. [92] lag unter Verwendung der Gegenstromelektrophorese bei gesunden Schwangeren der AFP-Spiegel unter 250 ng/ml. Radioimmunologisch konnten erste AFP-Spiegel schon zwischen der 7. und 14. SSW gemessen werden [179, 180, 380]. Die AFP-Konzentration bei gesunden Schwangeren lag im I. Trimenon bis 40 [380] bzw. 120 ng/ml [357, 382, 388], im II. Trimenon bis 360 [357, 380, 382, 388] und im III. Trimenon bis etwa 550 ng/ml ([357, 382, 388], Tabelle 3). Nach einem maximalen Anstieg zwischen der 35. und 38. SSW wurde ein allmfihliches Absinken bis zur 42. SSW auf einen mittleren Wert um 80 ng/ml und auf einen nicht mehr meBbaren Wert 20 Tage post partum beschrieben [179]. Nach Untersuchungen bei Mehrlings- [129] bzw. Zwillingsschwangerschaften [178] liegen die normalen AFP-Spiegel der Mfitter fiber dem Median der entsprechenden Wel~e bei einfachen Schwangerschaften, wobei sie bei Vierlingen die 97,5 Perzentile fiberschreiten. Bei pathologischer bzw. Risikoschwangerschaft kommt es im Zusammenhang mit einem ,,fetal distress"-Syndrom zu einem signifikanten AFP-Konzentrationsanstieg z.T. bis auf Werte fiber 1000 ng/ml [336, 339, 38t, 382, 387, 388], ferner mit Ausnahme einer Beobachtung [153] auch bei fetalen MiBbildungen wie z.B. Anencephalus [70, 72, 228, 229, 374, 160], Meningomyelozele [291], spina bifida [72, 229, 460] bzw. allgemein bei Neuralrohrdefekten des Feten [375] sowie auch bei Rh-immunisierten

Schwangeren vor allem bei konsekutivem Fruchttod [386] mit zwei Ausnahmen [454, 349], bei denen fiber gestationsnormale Werte berichtet wurde. Exzessive AFP-Spiegel treten beim fetalen Tod in utero auf [381,387] und k6nnen, vor allem bei spina bifida und Anencephalus, spfiter unter die SSW-Norm absinken [460]. Untersuchungen bei Patientinnen mit drohendem Abort im I. Trimester ergaben keine signifikanten Unterschiede, wohl jedoch im II. Trimester,

wo nur 2 von 12 Patientinnen mit normalem AFPSpiegel, aber 10 von 12 mit erh6htem Spiegel einen AbolXerlitten.AFP-Spiegelvorund nach legalem Abort im I. Trimester waren relativ unauffallig, im III. Trimester fiberwogen postoperativ erh6hte Spiegel [380]. Aufdie Bedeutung der AFP-SpiegelkontroUe wahrend der Schwangerschaft zur Differentialdiagnose gegenfiber Blasenmole oder dem Chorioncarcinom wurde yon Seppalfi u.Mitarb. [379] hingewiesen: erh6hte Spiegel zwischen der 6. und 26. SSW dfirfen als Zeichen der Anwesenheit des Feten gelten, wahrend der HCG-Nachweis lediglich Ausdruck trophoblastischer Aktivitat ist. Die Kombination nicht erh6hter AFPSpiegel nach der 12. SSW bei positivem HCG und vergr6Bertem Uterus ist verdachtig auf einen fehlenden Feten und spricht far Blasenmole oder Chorioncarci-

Bei Verdacht auf Anencephatie oder auch spina bifida wurde von Brock und Sutcliff [69] sowie anderen [18, 70, 71, 114, 153, 228, 341] die frfihzeitige (z.B. vor der 28. SSW, [153]) AFP-Bestimmung im Fruchtwasser empfohlen. Sie soll bei diesen Mif3bildungen im Gegensatz zu gedeckten Lasionen [153] durch ein vermehrtes Durchsickern bzw. eine Transsudation von fetalem Serum-AFP via Cerebrospinalflfissigkeit (spina bifida) oder durch den Plexus chorioides (Anencephalus) bzw. durch direkte Passage und nicht durch eine vermehrte Bildung durch den Feten [375] hervorgerufen werden. Normale Fruchtwasserspiegel (Tabelle4) fielen im Mittel yon z.B. 8/ag/ml (13.18. SSW, [114]) fiber 425 ng/ml (32.-34. SSW, [388]) und 164 ng/ml (37./38. SSW, [388])nach 87 ng/ml (41 ./ 42. SSW, [388]) ab und zeigten eine Korrelation zum Gestationsalter. Nach einer anderen Untersuchung [378] lagen im II. Trimenon die AFP-Fruchtwasserspiegel zwischen 2800 und 26000 ng/ml, niedriger als im fetalen Urin (5900-48000 ng/ml). Im III. Trimenon schwankten sie zwischen 15 und 535 ng/ml und im Neugeborenen-Urin zwischen 2,5 und 21 ng/ml. Alle Fruchtwasserspiegel vor der 37. SSW lagen bei 185 ng/ml oder h6her [378]. Bei fetalen St6rungen wurde eine AFP-Spiegelerh6hung im Fruchtwasser mit exzessivem Anstieg bei intrauterinem Fruchttod (bis maximal 48 000 ng/ml) gefunden, wobei der Anstieg gegenfiber dem mfitterlichen Serumspiegel viel frfihzeitiger und starker zu verzeichnen war [375, 388]. Erwahnt sei auch eine Mitteilung fiber einen stark erh6hten Fruchtwasserspiegel trotz nonnalem mfitterlichen Serum-AFP-Spiegel bei einem Kind mit kongenitaler Oesophagusatresie [385]. Die sonst gegen Ende der Schwangerschaft zu beobachtende AFP-Abnahme im Fruchtwasser bleibt bei Fallen von fetaler Erythroblastose aus oder verwandelt sich sogar in einen Anstieg [386].

Radioimmunologische AFP-Bestimmungen im Urin von Schwangeren [179] zeigten bei gesunden Frauen keine mel3bare Konzentration, wfihrend bei einer Patientin mit deutlicher Albuminufie (12 mg/mt) ein Wert yon 12 ng/ml (Serum : 63 ng/ml, 35. - 36. SSW) gefunden wurde. Auf eine m6gliche klinische Bedeutung von AFPBestimmungen in uterinen Blutungen, bei denen hohe AFP-Spiegel bei drohendem und sp/iter eingetretenem Abort signifikant h6her lagen als bei Austragung bis zum Termin, wurde von SeppN/i [383] hingewiesen.

Besondere Bedeutung kommt auch der AFP-Bestimmung im Neugeborenenserum zu, wobei die AFPKonzentration im umgekehrten Verhaltnis zur Reife des Neugeborenen stehen soll ([53, 174, 187, 225, 249,

Immunit~t gegen AFP Von den meisten Untersuchern [23, 183, 338, 339, 357, 382, 388] konnten im Serum yon Schwangeren oder Patienten keine AntiAFP-Antik6rper nachgewiesen werden. Im Gegensatz dazu steht eine Mitteilung/iber Antik6rperbildung bei einer Patientin mit AFPund AuAg-positiver akuter Virushepatitis bei gleichzeitig nachweisbarer Kryopr/icipitatbildung [116]. Eine passive Immunisierung gegen fetales AFP durch Injektion yon Anti-AFP-Serum in schwangere Kaninchen und Ratten bzw. befruchtete Hiihnereier [403, 409], die zu hoher fetaler Mortalit/itsrate, Dottersacksch/idigungen und fetalen MiBbildungen t'~ihrte, gab zu Vermutungen AnlaB, daB einige kongenitale MiBbildungen und Aborte beim Menschen durch mfitterliche Isoimmunisierung gegen AFP bedingt sein k6nnten [403]. Tsukada u.Mitarb. [439, 440] konnten nach Verabreichung von dekomplementiertem Pferde-Anti-AFP-Serum auf kultivierte AFPpositive Rattenhepatomzellen (Ascites-Hepatom AH 66) eine cytotoxische Wirkung auf die meisten Zellen feststellen, w/ihrend Zellen, die nur geringgradig AFP bildeten, selektiv i~bertebten.

Von den meisten Autoren [53, 174, 187, 249, 305] wurde eine signifikante Korrelation (r zwischen -0,57 und 0,89) zwischen dem AFP-Serumspiegel und dem Gestationsalter beschrieben, ebenfalls zwischen Gestationsalter und dem Logarithmus des Quotienten aus IgG- und AFP-Konzentration (r=0,861, [174]). Im allgemeinen P,illt der AFP-Spiegel schon in den ersten 4 - 5 Std post partum signifikant und dann wesentlich langsamer ab [187].
