NEW EVIDENCE-BASED CONTINUING EDUCATION RESOURCE AVAILABLE FROM COCHRANE AND WILEY

FROM COCHRANE DATABASE OF SYSTEMATIC REVIEWS (CDSR) ISSUES 10-11, 2013

The Cochrane Collaboration, an international and independent producer of systematic reviews of health care research, has partnered with John Wiley & Sons, Inc., a publisher of scientific and health care books and journals, to offer a new opportunity for evidence-based health care continuing education (CE). The goal of this partnership is to help clinicians use Cochrane evidence reviews to improve their practices. The Cochrane Collaboration has been in existence for 20 years and includes 31,000 individuals in 120 countries who produce reviews to promote evidence-based health care decisions.1 Now a sampling of information from these high-quality systematic reviews will be available to clinicians for CE via Cochrane Learning.2 Cochrane Learning is housed on the Wiley Health Learning Web site. Users can register at no charge, and once registered and logged in, they will find several featured CE programs available at no cost and additional selections available for a fee. Users may begin a CE program, save, and return at another time to complete the program. Completed programs and those in progress are stored on the site. All CE is accredited by the Accreditation Council for Continuing Medical Education. Additional resources will be launched in 2014.3 Readers of this Journal also may be interested in other CE resources via Wiley Health learning. Using the Browse By button, users may select a listed organization, practice area (such as nursing, midwifery, obstetrics & gynecology), or journal title and link to other CE resources specific to the selection. Again, some resources are featured and available at no cost and others are provided at a nominal fee. Continuing education offered by the Journal of Midwifery & Women’s Health and the Journal of Obstetric, Gynecologic, & Neonatal Nursing are among the resources included.

Featured Review: Long B, Liu FW, Bristow RE. Disparities in uterine cancer epidemiology, treatment, and survival among African Americans in the United States. Gynecol Oncol. 2013;130(3):652-659. The purpose of this review was to examine the literature and identify clinical, biologic, and socioeconomic areas in which there are disparities in endometrial cancer outcomes between African American and white women and how these disparities can be addressed. Previous researchers described differences in histopathology and health care access between these 2 populations. The current authors did not present the full search strategy. They identified a total of 42 related articles and reviewed a final set of 24 that specifically addressed differences between African American and white women. These studies included 366,299 cases of endometrial cancer: 337,597 in white women and 28,702 in African American women. Among the reviewed studies, the incidence of endometrial cancer was less in African American women than in white women; however, the mortality from endometrial cancer was much greater for African American women. This difference (1.5 times greater in one study to more than 3 times greater in another study) was attributed to the increased incidence of aggressive subtypes of endometrial cancer among African American women. In 6 studies (only one prospective), researchers documented that African American women were more likely to have more aggressive endometrial cancer subtypes and higher-grade tumors than white women. African American women also had more aggressive tumors than white women at the same stage. Certain gene mutations with more favorable prognoses may be less common in African American women; however, research results are conflicting. Researchers examining estrogen therapy following surgical treatment for endometrial cancer suggested the possibility of differences in estrogen metabolism between African American and white women as a factor contributing to the disparity in mortality rates. These differences may point to possible treatment regimens for African American women. Socioeconomic disparities such as poverty, less educational attainment, and less access to health insurance and primary care may also contribute to disparities in endometrial cancer outcomes between African American and white women. Most studies were retrospective with smaller samples. Using multivariate modeling, access to care and income 211

variables were associated with some of the differences in survival outcomes. These factors may be more relevant for less aggressive endometrial tumors for which there is greater opportunity to make a difference based on the timing of treatment. Although comorbid conditions may affect survival, this has not been clearly demonstrated in the research to date. Differences in surgical treatment at various stages of the disease have been observed in the past, and African American women were less likely to receive the standard of care than white women. However, in more recent studies, researchers have found similar care standards for African American and white women. Comment: The observed disparities in survival rates following diagnosis of endometrial cancer between African American and white women are likely multifactorial. Continued research focusing particularly on histologic, socioeconomic, and treatment factors is needed. New molecular research approaches are promising. Improved access to health care in the United States as well as appropriate education among all women and primary care providers related to symptom recognition and appropriate referrals should help make a difference. Featured Review: Goyal NK, Teeters A, Ammerman RT. Home visiting and outcomes of preterm infants: a systematic review. Pediatrics. 2013;132(3):502-516. This systematic review was undertaken to review evidence about home visiting and the effects on preterm infants. The authors observed that mixed results have been reported in recent reviews of home visiting programs for preterm infants and that additional resources have been made available to expand home visiting programs through the Affordable Care Act. The authors specifically examined timing of enrollment in home visiting, program characteristics, and additional social risk factors that may affect outcomes for preterm infants. The authors searched articles published between 1980 and 2012 on home visiting programs initiated prenatally or following birth. Studies of term and preterm infants were included if outcomes for preterm infants were reported separately. Each article was assessed for quality and reviewed by 2 reviewers. Thirty-eight published articles representing 17 studies were selected for inclusion. These studies included 2983 infants with a median study sample size of 100. Interventions were provided by nurses (8 of 17 studies), developmental specialists, trained paraprofessionals, and a combination of health care providers. Frequency of visits and duration of the program varied among studies. Some programs were initiated during the postpartum hospital period, and some included community-based group sessions as well as home visits. The authors identified 5 outcome domains reported in the reviewed studies: infant development, morbidity and health care utilization, abuse and neglect, parent-infant interaction, and growth and nutrition. A meta-analysis of the Bayley Mental Development Index (MDI) demonstrated a statistically significant overall positive effect for home-visited infants compared to control infants. However, significant heterogeneity was observed, possibly related to visit frequency. A statistically significant effect size was also demonstrated for the Home Observation for Measurement of the Environment (HOME) Inventory scores for infants who received home visits compared to control infants. In one study, the authors described an im212

proved effect on growth at 4 and 8 months. In another, the authors found improved growth at 8 years for infants in the intervention group who weighed less than 1500 g at birth compared to control infants. In 2 studies, researchers measured abuse and neglect; neither demonstrated a significant difference. Most studies were randomized controlled trials; loss to follow-up was the greatest weakness observed. Comment: Results of this meta-analysis demonstrate positive effects of home visiting on parent-infant interaction and possibly on infant development. Evidence for an effect on morbidity, abuse and neglect, and growth is limited. The authors recommend that future researchers examine the effect of home visiting on premature infants from socially disadvantaged backgrounds and consider targeted interventions for that subgroup in an effort to enhance the benefits of home visiting programs.
