Background: The influence of occlusal factors on the temporomandibular joint's morphology is controversial. Previous studies have been based on conventional X-rays. Computer tomography (CT) can help us shed new light on this issue. Aim: This study focused on defining and comparing—in CT images of the temporomandibular joint—the condyle's position in that joint's cavity in clinically asymptomatic patients with “normal occlusion” and those with malocclusion in classes I, class II/1, and class III. Methods: In each group, the authors included 30 patients with CT images of the temporomandibular joints of asymptomatic individuals with “normal occlusion” and of persons with malocclusion categorized in class I, II/1, and III. Permanent dentition had erupted and was intact in all individuals except for the wisdom teeth. None of them presented forced bite, any apparent facial asymmetry, or an open bite, and except for the Class III group, crossbite. The group with “normal occlusion” consisted of individuals with Class I occlusion, a normal overjet and overbite, as well as canine guidance without side interferences on lateral excursion. The CT images were taken with the patients in maximum intercuspidation and oriented to Frankfurt horizontal. Measurements were taken of the anterior joint space at the narrowest point between the condyle and tuberculum articulare, and of the posterior joint space at the narrowest point between the condyle and rear wall of the fossa. Condylar positions were determined by assessing the relation of the condyle to the anterior and posterior joint space. Central position is defined by a ratio of 1:1. This relationship was statistically evaluated on the right and left sides in all groups as well as the distribution in each group.

Hintergrund: Der Einfluss okklusaler Faktoren auf die Morphologie des Kiefergelenks wird kontrovers diskutiert. Bisherige Studien basieren auf konventionellen Röntgenbildern. Mit Hilfe der Computertomographie (CT) kann die Thematik neu bearbeitet werden. Ziel: Ziel der vorliegenden Studie war es, anhand von Computertomographien des Kiefergelenks die Position der Kondylen in der Gelenkgrube bei klinisch asymptomatischen Individuen mit „Normalokklusion” und mit Malokklusionen der Klasse I, Klasse II/1 und Klasse III zu bestimmen und zu vergleichen. Methode: Jeweils 30 CT-Bilder der Kiefergelenke von symptomfreien Individuen mit „Normalokklusion” und mit Malokklusionen der Klasse I, II/1 und III wurden herangezogen. Bei allen Individuen waren mit Ausnahme der Weisheitszähne alle permanenten Zähne durchgebrochen und vorhanden, es bestanden keine Zwangsbissführung, keine offensichtliche Gesichtsasymmetrie, kein offener Biss und, mit Ausnahme der Klasse-III-Gruppe, kein Kreuzbiss. Die „Normalokklusionsgruppe” bestand aus Individuen mit Klasse-I-Okklusion, normalem Overjet und Overbite sowie mit Front-/Eckzahnführung ohne Balancekontakte. Die CT-Bilder wurden in maximaler Interkuspidation mit horizontaler Ausrichtung der Frankfurter Horizontale aufgenommen. Gemessen wurden der vordere Gelenkspalt an der engsten Stelle zwischen Kondylus und Tuberculum articulare und der hintere Gelenkspalt an der engsten Stelle zwischen Kondylus und der Rückwand der Fossa. Die Position der Kondylen wurde durch das Verhältnis von vorderem zu hinterem Gelenkspalt ermittelt. Eine zentrische Position ist bei einem Verhältnis von 1:1 gegeben. Results: The authors seldom detected a central condylar position in the group of patients with “normal occlusion” or those with malocclusions. All the groups displayed an anterior joint space that was less wide than the posterior joint space. All the groups, however, presented primarily symmetrical decentralization, indicating an off-center but bilaterally symmetrical condylar position. The greatest decentralization was identified in the Class II group, the least in the group with “normal occlusion” — a difference that was significant on the left side. Conclusion – An off-center condylar position of minor width in the anterior joint space is physiological, especially when the relationships are bilaterally symmetrical. – Individuals with Class II/1 but no clinical symptoms display the most pronounced deviation in condylar position in the ventral direction, while those with “normal occlusion” reveal the least deviation. Ergebnisse: Eine zentrische Kondylenposition war sowohl in der Gruppe mit „Normalokklusion” als auch in den Malokklusionsgruppen selten. In allen Gruppen war die vordere Gelenkspaltbreite geringer als die hintere. Die Dezentralisation war jedoch in allen Gruppen weitgehend symmetrisch, was eine zwar azentrische, aber beidseits symmetrische Kondylenposition anzeigt. Die stärkste Dezentralisation lag in der Klasse-II-Gruppe vor, die geringste in der Gruppe mit „Normalokklusion”. Dieser Unterschied war auf der linken Seite statistisch signifikant. Schlussfolgerungen: – Eine azentrische Kondylenposition mit geringerer Breite des vorderen Gelenkspalts ist physiologisch, besonders wenn beidseits symmetrische Verhältnisse vorliegen. –Klasse-II/1-Individuen ohne klinische Symptome zeigen die stärkste Abweichung der Kondylenposition nach ventral, Individuen mit „Normalokklusion” die geringste. Interessenkonflikt. G. Göz gibt an, dass kein Interessenkonflikt besteht.
