The history of the patient goes to show that he was long subject to hernial protrusion, but it was always reducible by slight manipulation Three days before his admission it came down rather suddenly and refused to yield to his manipulatory efforts Distinct impulse was communicated to the lingers on conghing, and gurgling was perceptible, while manipulation to of the seemed relieve some of its distension. Gentle taxis was tried without any good result. The tumour did not cause much inconvenience to the patient except a sense of uneasiness and slight pain in the tumour and attended with colickly pains in the abdomen. lotions locally?ice not The case thus continued for a month during which time taxis was occasionally tried, but with as little success as before. On the 2nd of October symptoms of strangulation suddenly set in : the bowels, which were hitherto acting freely, became constipated, vomiting became incessant and the tumour painful. There was no reason to wait, the above symptoms in an already existing irreducible hernia being more than indicative of immediate operative interference. The patient was accordingly placed on the 3rd under chloroform and gentle taxis tried first while under it, but the tumour not yielding, the operation of herniotomy was proceeded with, and the constricting band outside the sac being incised the bowel was immediately reduced. The wound afterwards healed up by first intention.
